- Bachelor’s Degree in Pharmacy
- Current unencumbered CA Pharmacy licensure
- Current BLS certification for Healthcare Providers
- One (1) year current inpatient Pharmacist experience or completion of a Pharmacy Practice Residency
- Completion of a Pharmacy Residency program
- Doctor of Pharmacy (Pharm.D.)
