Job Description
Overview:
A licensed practical nurse bears full responsibility for the quality of health care he/she provides to the residents at Seven Hills at Groton. A licensed practical nurse may delegate nursing activities to assigned health care personnel. The delegating licensed practical nurse shall bear
full responsibility for making an appropriate assignment, adequately teaching, directing and supervising the delegate(s), as well as the outcome of that delegation; all within the parameters of his/her generic and continuing education and experience. Provides quality care to children and young
adults in accordance of the Nurse Practice Act of Massachusetts

Responsibilities:
Delivers competent nursing care.

Conveys through nursing practice a clear sense of the role and identity.

Contributes to individualized plan of care in which needs are identified and achievable goals are set.

Implements nursing care plan in an organized manner; establishes and reorders priorities as needed. Evaluates outcomes of basic nursing interventions. Incorporates the prescribed medical regimen into the nursing plan of care.

Deliver care that is consistently reflective of the standards of care at Seven Hills at Groton.

Administers medications and treatments safely according to established nursing practice.

Systematically assesses the resident’s basic health status, records and related health data. Recognizes changes or trends in patient status.

Participates in analyzing and interpreting data and making informed judgments as to the specific elements of nursing care, using appropriate resources and NP/MD consultation.

Participate in the health teaching required by the family and other responsible parties.

Collaborate, cooperate and communicate with other health care providers to ensure quality and continuity of care.

Delegates responsibilities for the performance of selected resident care activities to assistive personnel within the established delegation framework and provides appropriate supervision. Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.

Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities. Ensure that your assigned certified nursing assistants (C.N.A.’s) are aware of the resident care plans. Ensure that the C.N.A.’s refer to the resident’s
care plan prior to administering daily care to the resident.

Make a written and oral report/recommendation concerning the activities of you shift as required.

Admit, transfer and discharge residents as required.

Complete accident/incident reports as necessary. Appropriately notify/inform supervisory staff.

Perform administrative duties such as completing 30 day summaries and edits as necessary.

Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.

Receive telephone orders from physicians and record on the Physician’s Order Form.

Accurately transcribe physician’s orders to resident charts, Kardex, medication cards, treatment/care plans, as required.

Chart nurse’s notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Report all discrepancies noted concerning physician’s orders, diet, charting error, etc., to the Nursing Supervisor.

Sign and date all entries made in the resident’s medical record.

Ensure that an adequate supply of medications is on hand to meet the nursing needs of the residents. Properly order medications in a timely fashion per pharmacy guidelines.

Ensure that the narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift.

Review MAR for completeness of information, accuracy in the transcription of the physician’s order and necessary initials to indicate doses administered.

Dispose of drugs and narcotics as required and in accordance with established procedures.

Responds to complaints and grievances made by family or responsible parties. Report to nursing supervisor as necessary. Follow Seven Hills at
Groton grievance policy.

Participate in the orientation of new residents/family members to the facility.

Make rounds with physicians and actively participate in planning care for assigned Residents.

Arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures.

Qualifications:
Licensure to practice as an LPN in Massachusetts is required. Prefer experience as a Certified Nursing Assistant if new graduate.
