Terros Health is hiring a Patient Navigator at our 27th Avenue Health Center in Phoenix, AZ.
Full-Time, Employed
Location: 27th Ave/ Indian School Rd
Monday-Friday, 8am-5pm
No Weekends
Education/ Experience Requirements:
High School Diploma, GED within six (6) months of assuming this position Or Associates Degree in Medical and Healthcare
At Least One Year - Relevant Front Office Customer Facing and Focused Service Experience in Healthcare, Hospitality

Patient Navigator Duties Include:
- Greets individuals coming into the clinic in a professional and courteous manner, assists their needs promptly with compassion, in accordance with Terros Health values and utilizing Language of Caring.
- Provides superior customer service to patients checking in through the patient check-in kiosk. Educates patients in the use of the kiosks/iPads and assists as needed.
- Works to engage patients and their families, as well as others on services offered at Terros Health.
- Coordinates with the Practice Manager, Clinical Site Manager and/or Site Director to prevent and resolve on-site complaints.
- Maintains presence in the lobby of the clinic in order to help support patients and families, as well as other front office staff.
- Assists patients in connecting to resources based on the social determinants of health (SDOH). Skilled at uncovering patients’ needs, then following through with enlightening solutions.
- Provides health screenings when appropriate. Helps patients develop visit plans prior to seeing a provider.
- Gather and maintain patient demographics, ensuring accurate and complete information, in compliance with UDS guidelines. Encourage patients to address gaps in care.

Qualifications:
The applicant must have the minimum combination of education and/or experience:
- High School Diploma, GED within six (6) months of assuming this position Or Associates degree in medical and health care (e.g. healthcare administration, insurance billing and coding, psychology) or social work, human services, communication preferred.
- Have strong people skills- approachable, a good listener, and empathetic. Passionate about Terros Health’s mission and eager to share that passion with others.
- At least one- year relevant front office customer facing and focused service experience in healthcare, hospitality (i.e., meeting or conventions, hotel), or banking required. Front office experience in healthcare, behavioral health, dental or a medical office is preferred.
- Familiarity with health insurance eligibility and electronic health record management systems. Experience with NextGen preferred.
- Intermediate level of competency in Microsoft Suite and web-based programs.
- Experience in managing a wide range of administrative, support related tasks. Must be well organized, flexible, able to multi-task, possess strong time management skills, attention to detail and enjoy working in a fast-paced, ever changing environment.
- Excellent written and verbal communication skills.
- Proven track record of delivering a high level of customer service in a healthcare setting.
- This role is a non-driving position. Must be 18 years of age and with less than two years’ driving experience or no driving experience.
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
- Must pass a TB Test.
