Under the direct supervision of the FSS Director, the Patient Care Coordinator is an essential member of the health care team. The position is responsible for promoting improved access and care management to meet the needs of patients while helping navigate effectively and efficiently though TVHC’s healthcare system. This position includes being a single point of contact to receive referrals from external health care agencies and TVHC’s primary care medical teams. The Patient Care Coordinator provides individualized assistance to ensure patients are informed of potential care options and have full access to all needed health and wellness services to ensure optimal health. This position also promotes timely and coordinated internal communications among clinic primary and dental care, behavioral health specialists, wellness program staff and patients. In order to maximize patient access and provider productivity, this position also collaborates with clinic staff, assists with scheduling patient appointments and provides referrals and linkages for community resources to support whole person health. The Patient Care Coordinator will collaborate with community partners and local agencies to establish health care services for those in need.
This position utilizes the electronic health care record and population management tools to collect data, communicate with providers to support collaborative care, improve health outcomes and program compliance. The Patient Care Coordinator plays a role in program design and implementation to ensure the highest utilization for optimum health outcomes in affected patient groups.
This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education and more.
Responsibilities:
- Accepts warm hand-offs between agencies and coordinates care for patients to facilitate integration of care.
- Fully knowledgeable of patient eligibility requirements for additional service and assist patients in navigating their care with both in-house and outside providers by facilitating scheduling of visits, identifying barriers to receiving care, and assisting with providing referrals to community and internal resources.
- Respond to patient inquiries and assist patients with the completion of paperwork.
- Develop relationships with health care providers within and outside the community health center.
- Identify patient barriers to care such as transportation, insurance, and language barriers.
- Refer patients as needed to additional resources to support engagement in care.
- Assist in helping patients arrive at scheduled appointments on time and prepared, coordinating transportation when needed.
- Using Electronic Health Record (EHR) system, track navigation services and contribute to clinic tracking workflows.
- Track appropriate patient-level quality outcomes for program monitoring and evaluation.
- Attend relevant stakeholder and advisory meetings to support overall program implementation and evaluation.
- Attend trainings and learning opportunities that expand content knowledge and best practices.
- Participates in, coordinates, and/or attends and conducts outreach at community events such as health fairs and community forums, as a representative of TVHC.
- Position is responsible for data collection and generation of reports as directed by the supervisor.
- Adhere to privacy and information security requirements (HIPAA) and other regulations. Performs functions, duties and/or services in compliance with regulatory agencies, contractual obligations and funding source requirements.

Qualifications:
- Associate or Bachelor’s degree in social work, psychology or relevant field, or one-two years work experience in a community clinic.
- Bilingual in English and Spanish required/multilingual ability.
- Experience working with or in Federally Qualified Health Center (FQHC) preferred.
