Position Summary:
The Patient Care Coordinator facilitates an excellent patient experience by providing the highest level of customer service to patients/family members, other healthcare entities and fellow employees both in-person and over the phone. This role is responsible for activities associated with administrative operations.
Duties include greeting and assisting patients in person and over the phone, call screening, schedule coordination, gathering/updating patient information and working with outside healthcare facilities to coordinate care and obtain necessary documentation.
Essential Responsibilities:
· Answers incoming calls in a professional and courteous manner, promptly responding to requests/inquiries or directing/triaging calls for clinical personnel
· Greets patients and visitors in a welcoming and helpful manner
· Obtains patient demographic information and accurately enters/updates EHR
· Schedules patient appointments and ensures that all required follow-up is complete
· Confirms future appointments following established guidelines
· Performs pre-visit chart audit reviewing for required documents/plan of care requirements
· Explains insurance requirements (prescription, PCP appointment, delivery receipts, etc.) to patients/families and tracks to completion making follow-up outreach as needed
· Collects documentation and patient payments as determined by Central Billing Office
· Documents all patient related activity in EHR in an accurate, thorough and timely manner
· Opens/sorts incoming mail
· Attends and participates in daily/weekly meetings
· Assists in marketing efforts to build relationships with local referral sources
· Performs other duties as assigned
Qualifications:
· Two years medical office working directly with patients
· EHR/EMR experience
Skills/Competencies:
· Excellent communication/interpersonal skills with the ability to interface with patients, physicians and coworkers in a tactful, informed and service-oriented manner
· Ability to prioritize workload and manage required follow-up
· Detailed orientation
