Occupational Summary

Position coordinates patient referrals to appropriate resources for the clinical operations in the Duke Smoking Cessation Program, part of the Supportive Care and Survivorship Center of the Duke Cancer Institute.

Work Preformed

Identify, coordinate, and facilitate patient referrals to face-to-face, telehealth, and other community resources for patients referred to Smoking Cessation Program. Contact patietns who are lost to follow-up to re-engage them in care.

Assist patients by coordinating patient assistance program resources and external resources, and communicate to Providers outcome of referral to assistance program.

Coordinate with other internal and external stakeholders, including research teams, financial care counselors, and referring providers, to facilitate patient care.

Monitor, track, and update information regarding referrals to the Program and maintain records for information concerning services rendered to be used for program evaluation.

Provide administrative support to Program Director, Manager, and providers by managing templates and schedules and other administrative work as assigned.
