Certified Home Care agency seeking full time Registered Nurses.

JOB RESPONSIBILITIES:

1. Utilizes knowledge of current nursing practice in assessment of physical and psychological needs of the patient.

2. Plans, implements and coordinates UAS/PRI assessments in the patients home.

3. Carries out physician orders per agency and regulatory requirements and within limits of practice.

4. Comprehensive documentation appropriately reflects patient care from admission to discharge utilizing all UAS/PRI assessment tools.

5. Participates in the agency’s quality assurance program.

6. Attends agency, team and discipline specific meetings and inservices as required.

7. Available during scheduled hours, including on-call rotation as assigned.

8. Serves as case manager in directing multidisciplinary patient’s plan of care.

9. Participates in and coordinates documentation of multidisciplinary communication between all appropriate individuals involved in patient’s care.

10. Conducts, communicates and documents physician contacts to all appropriate individuals involved in the patient’s care and insurance companies as requested.

11. Communicates patient information to manager on a regular basis and as needed as appropriate.

12. Notify manager of all scheduling needs for patient visits.

13. Provides quality care within guidelines of agency policy and procedures as well as local, state and federal rules and regulations.

14. Enhances professional development by participating in seminars, workshops and meetings in accordance with the continuing education requirements established by state licensure/laws of New York State.

15. Other responsibilities, as deemed necessary by the Director for Patient Services for the efficient operation of the agency, which falls within his/her professional competence and scope of clinical practice.
