Job Description

Under the direction of the Manager of the Winchester Chest Clinic working collaboratively with the Interstitial Lung
Disease Program Director and the Ambulatory Medicine Leadership assumes clinical and administrative
responsibilities for the Interstitial Lung Disease Program at YNHH. The Nurse Coordinator maintains oversight ,
coordination, and evaluation of the Interstitial Lung Disease Program supporting education, clinical practice, and
quality improvement. Serves as a role model for inter-professional communication and exemplifies organizational
leadership. The Nurse Coordinator for the Interstitial Lung Disease Program is accountable for strategic program
development, implementation, evaluation, and provides oversight and coordination of all elements of the designation
as a Pulmonary Fibrosis Foundation Care Center Network, as a Scleroderma Center by the Scleroderma Foundation
and as a WASOG-FSR Sarcoidosis Center of Excellence. The Nurse Coordinator maintains communication with
patients, families, staff and providers in order to ensure coordination of patient care across the continuum .

initiatives, and processes associated with the Interstitial Lung Disease Program at YNHH.
1.1 Provides vision and leadership in a collaborative environment that offers job satisfaction , recognition, and
stimulates innovative thinking to accomplish goals and objectives. Collaborates with leadership in the
development of objectives, goals, and strategic plans for the Interstitial Lung Disease Program and
develops actions plans to accomplish set goals and plans.
1 .2 Collaborates with the Program Director and leadership in the development of best practices, standards
of care, benchmarks, and evidence-based treatment guidelines and goals.
1 .3 Manages and provides administrative oversight of the Interstitial Lung Disease Program and specific
programs within the Interstitial Lung Disease Program including the sarcoidosis and scleroderma
programs. Establishes systems and processes that ensure effective program operations .
1 .4 Implements and actively supports change through open communication and use of change management
skills.
1 .5 Collaborates with the Program Director and Leadership team to ensure ongoing growth and development
of the program.
1 .6 Leads multidisciplinary team utilizing available methodologies to achieve established performance goals.
In collaboration with the Program Director, clinical leadership, physician leaders, nursing, and pharmacy
provides oversight in the development of Interstitial Lung Disease management protocols and guidelines.
1 .7 Serves as a clinical leader, resource, and role model for the Interstitial Lung Disease Program.
In collaboration with the medical team, provides continuity of care to patients within the inpatient and
outpatient setting in order to achieve optimal patient outcomes and patient satisfaction.
1 .8 Participates in rounds with the Attending Physician and clinical team ; collaborates with the patient's
nurses and members of the health care team regarding current medical plan of care. Attends
interdisciplinary meetings, discharge planning rounds, and family meetings as a representative of the
Interstitial Lung Disease Program. 1.9 Maintains ongoing communication with team members and in collaboration with the attending
physicians and fellows conveys pertinent medical information to primary and referring physicians.
1 .10 Maintains pre-admission contact with patients, families and referring physicians. Manages incoming
referrals, coordinates initial patient contact and complies list of presented patients in collaboration with
medical team. Conveys pertinent medical information in collaboration with the attending physicians and
healthcare team to patients primary physicians, referring physicians, and insurance providers.
1 .11 Provides direct patient care during clinic visits in collaboration with the Attending Physician , fellows,
residents and nursing staff.
1 .12 Maintains inpatient follow up and assesses patient needs throughout hospitalization; collaborates with
the appropriate individuals to plan and facilitate transitions of care and coordination of follow up visits.
1 .13 Supports and educates patient/family regarding diagnostic and therapeutic experiences to minimize
anxiety and ensure adherence to medical plan; provides warm handoffs as appropriate and maintains
contact with patient/family and health care team as patients move along the continuum.
1 .14 Responds to patient/family telephone contacts according to assessment findings, physician direction
and/or established standards of care.
1 .15 Facilitates the ongoing care of patients following discharge through consultation with patients, families,
and home care agencies. Maintains responsibility for post discharge phone calls and follows up as
appropriate. Serves as a resource to assist patients with procurement of medications.
1 .16 Functions as a clinical resource for residents and nursing staff maximizing opportunities in the clinical
setting for educational opportunities.
1 .17 Collaborates with the Interstitial Lung Disease Program laboratory and clinical trials personnel to
enhance the program's research mission.
1 .18 Is responsible for managing the weekly multidisciplinary Interstitial Lung Disease discussions, including
maintaining a list of cases to be discussed, documenting and recording the discussion.
1 .19 Demonstrates exceptional knowledge and skills that reflect expertise in communication, customer service ,
professionalism, problem solving, resource management, teamwork, and leadership.
2. Ensures a professional work environment and maintains responsibility for professional practice and
individual professional development.
2 .1 Oversees the clinical development, management, quality, and outcomes of the Interstitial Lung Disease
Program.
2 .2 Maintains excellent understanding of medical knowledge regarding management of interstitial lung
disease utilizing evidence based practice.
2 .3 Maintains excellent understanding and knowledge of therapies related to interstitial lung disease.
2 .4 Proficient regarding the approval, initiation, and maintenance of all interstitial lung disease therapies.
Active in regional, national, and international interstitial lung disease programs; participates in
organizing committee work, task force membership, and presentations.
2 .5 Facilitates interstitial lung disease support group for patients and families. Participates in and
represents the Interstitial Lung Disease Program in patient- and family-sponsored events.
2 .6 Responsible for meeting the Pulmonary Fibrosis Foundation, Scleroderma Foundation, and the WASOG
FSR Sarcoidosis Center of Excellence requirements of a Nurse Coordinator. 2 .9 Develops staff through coaching, mentoring, and delegation.
Demonstrates positive and professional interpersonal relations with multidisciplinary team to ensure
productivity and positive employee relations.
2 .10 Supports and advances the vision and strategic direction of the Interstitial Lung Disease Program.
Maintains excellent understanding of available clinical trials and translational research. Educates
patients on participation in relationship to clinical care.
2 .11 Serves on hospital committees, attends conferences, and professional meetings as required.
2 .12 Represents Medicine Service and functions as a role model and leader.
2 .13 Provides leadership and direction for team process.
Represents the Interstitial Lung Disease Program to all major constituencies including hospital
administration, departments, external agencies, and organizations.
2 .14 Supports and exemplifies the principles of a High Reliability Organization.
In collaboration with leadership, maintains responsibility for the implementation of the Interstitial Lung Disease
quality improvement program.
3. Maintains oversight of the coordination and monitoring of interstitial lung disease processes at YNHH
appropriately incorporating identified opportunities into performance improvement initiatives.
3 .1 Monitors outcomes through systematic documentation of strengths, opportunities, and program
successes. Collaborates with leadership team to identify and develop comprehensive plans in response
to identified strengths and opportunities.
3 .2 Participates in care and performance improvement activities for effective review of patient care and
operations.
3 .3 Ensures data is pertinent and valuable for process and performance improvement.
Collaborates in the implementation of action plans for performance improvement projects based on data
and outcomes.
3 .5 Maintains and prepares for compliance readiness related to ongoing Joint Commission and State
surveys and certifications.
3 .6 Provides leadership, expertise and support in planning, promoting, designing, and ensuring delivery of
education to nurses, physicians, residents, support staff, patients, families, and community based on best
practice and YNHH's strategic goals and initiatives.
4. Communicates and works with physicians and clinical staff to assess educational needs of patients ,
families and community.
4 .1 Functions as a resource for providers and community agencies.
Provides oversight and coordination of all elements of the Interstitial Lung Disease Association accreditation
process.
5. Responsible for collecting and analyzing data related to interstitial lung disease measures and metrics.
Assists with the submission of data to the certifying bodies for Interstitial Lung Disease, Scleroderma,
and Sarcoidosis accreditation. 5.1 Troubleshoots data and data flow to complete projects or initiatives in accordance with timelines and
project plans.
5 .2 Coordinates and provides community education and support groups related to interstitial lung disease
and treatment.Qualifications: EDUCATION:
Bachelor's Degree in Nursing is required. Master's Degree is preferred.
Current in BLS.
EXPERIENCE:
Five to eight years experience as a nurse in an acute care setting or critical care.
LICENSURE/CERTIFICATION:
Current CT State Nursing license required.
SPECIAL SKILLS:
Knowledge of pulmonary physiology and interstitial lung disease.
Must have excellent clinical and decision making skills and understanding of organizational processes and
dynamics. Must have high level of personnel management, ability to make independent judgments and implement
required changes. Must be a change agent and possess excellent interpersonal, communication, organizational, and
leadership skills. Must be self-motivated, goal oriented, and able to work independently. Must have the ability to
work cooperatively and effectively with physicians, clinical staff, hospital departments, and outside organizations
utilizing effective verbal and written skills. Must be proficient with computer based applications. Ability to manage
multiple priorities and meet specific deadlines.
ACCOUNTABILITY
Coordinates the care of the interstitial lung disease patient across the continuum, from initial referral to post
procedure and outpatient visits as well as directing referrals externally to other departments if more appropriate .
Serves as a clinical consultant to patients, families, hospital and medical personnel regarding interstitial lung
disease. Responsible for assisting program leadership with the collaboration, integration, continued development, and growth
of the Interstitial Lung Disease Program. Responsible for directing nursing and clinical support staff related to
Interstitial Lung Disease practice guidelines. Implements and monitors process improvement and ensures
compliance to Interstitial Lung Disease accreditation standards.
COMPLEXITY
Requires management of a complex patient population and assists with administrative and operational
responsibilities of all aspects of the Interstitial Lung Disease Program.
Additional Information:BSN required, Masters preferred. Knowledge of pulmonary physiology and Interstitial Lung Disease (ILD) process. Must have excellent clinical and decision making skills and understanding of organizational structure and dynamics. Must be a change agent and possess excellent interpersonal, communication, organizational and leadership skills.
