UW Medical Center - Montlake has an outstanding opportunity for a Nurse Care Coordinator. This is a temporary position for up to 9 months. The Nurse Care Coordinator serves as a member of the University of Washington Medical Center Social Work and Care Coordination department, working as part of a Social Work and Nursing Care Coordination team, with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. This position works closely with all members of the interdisciplinary health care team, insuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient.
SCHEDULE
This is a temporary position for 6-9 months. Position is full-time days Monday – Friday.
