Job Description

Join CSO's innovative Community Partner (CP) Program! Be part of our new team of caring and skilled professionals as a Nurse Care Coordinator.

Our multidisciplinary team consists of nurses, licensed clinical behavioral health specialists, Bachelor level care coordinators, and peer partners. Together we provide individualized support to adult MassHealth members who have complex medical and behavioral health needs. We coordinate communication between behavioral health, substance use, and medical care providers, and connect people to services in the community to help them meet their self-identified health and wellness goals.

Qualifications:

  • Registered Nurse or Licensed Practical Nurse
  • Experience working with adults who have behavioral health or substance use challenges
  • Ability to establish trusting relationships through compassion, empathy and insight
  • Demonstrated sensitivity to the needs and perspectives of individuals from diverse cultural or linguistic backgrounds
  • Experience in primary care or home visiting preferred
  • Computer proficiency required including ability to work with email, electronic health record and word processing software
  • Valid driver's license and reliable transportation
