Please continue!YOUR CONTRIBUTION TO OUR SUCCESS!: The Non-Medical Case Manager blends the mission of AHF with Psychosocial Case Management, and will work with patients so that the eligibility process to utilize Ryan White services is completed and remains current. As a Non-Medical Case Manager, you will understand the social deterrents of this population, and a thorough understanding of the Ryan White Care Act Program within the local EMA. (Pinellas County, FL). With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White patient receives the support they require to maintain eligibility, access medical care, medication, and available resources. You will provide outreach and education so patients understand the Ryan White services available to them and the reasons essential to remaining eligible for the program. Collaboration with your Healthcare Center and the Medical Case Manager is essential as you work with newly diagnosed patients, patients lost to care, and patients currently in care but needing eligibility. In this way, you will support the patient’s ability to maintain better/improve their health status so they can work to achieve their healthcare goals.
Combined with a thorough understanding of the Ryan White Care Act Program within their specific area, the Non-Medical Case Manager collaborates with the Medical Case Manager, Healthcare Center Staff, and Providers so that every Ryan White patient receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood.
Utilizing your case management experience, you will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for the uninsured HIV/AIDS patients - to our most vulnerable population. Your eligibility completion and support will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.
