Delivering H.O.P.E. Human Services, Inc. (DHHS) was founded in 2009 and is located on the Southside of Richmond. At Delivering H.O.P.E., we provide Mental Health Skill-Building Services (MHSS), Psychosocial Rehabilitation, Intensive In-Home (IIHS) and Crisis Stabilization Services.

We are currently seeking experienced Qualified Mental Health Professionals (QMHP-A & QMHP-C) to provide Mental Health Skill-Building and Intensive In-Home Services.
Responsibilities and Duties:
- All candidates must have passion, heart and a desire to work in the Human Service Field.
- Maintain a caseload of 2-3 clients per week and provided 9 to 15 hours per week.
- Candidates must be able to write progress notes that correlate with services outlined within the individualized Treatment Plan ( ISP's).
- Prepares written service plan, incorporating input from clients, involved service providers, and wraparound team members, and clearly identifying problem areas and needs, strategies, and service objectives
- Responds to crisis situations with twenty-four hour a day availability
- Facilitates wraparound team meetings to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports.
- Collaborates and coordinates with other case involved service providers and professionals Links clients with additional services, such as health services, recreational activities, housing, financial resources, etc.
- Additional responsibilities are also needed to encourage and promote continuity of care of clients.

Qualifications and Skills
All candidates must be registered and approved as a QMHP-A or a QMHP-C by the Board of Health Professions. Candidates considered must have a Bachelor's Degree or higher in Social Work, Sociology, Human Services, Psychology, Counseling, or Special Education and at least 1 year of experience work with adults and children with a mental health diagnosis. Additional qualifications include a valid driver's license, reliable transportation, current automobile insurance, and successful completion of a criminal background check.
