Mental Health and Substance Use Disorder Case Manager
Basic Function
Responsible for assisting with tracking medication and intake documents for prescribers, scanning documents in the electronic record system, telephone calls to consumers, and community-based face to face outreach contacts for those enrolled at the Behavioral Health Services of Racine County.
Essential Duties
- Screening members of target population.
- Basic risk reduction messages (risk hierarchy).
- Distributes materials to support risk reduction.
- Develops a case plan with consumers.
- Tracks no show medication injection appointments.
- Facilitates a group for consumers consistently not showing for appointments.
- Provides referrals to resources needed by the consumers.
- Responsible for linking clients to other program components.
- Responsible for follow up phone calls, home visits, and attempts to locate consumers (outreach and clinic) for engagement in services.
- Responsible for documenting all contacts with the consumers.
- Participates in professional staffing and meetings as required.
- Maintains records in compliance with Racine County, Wisconsin and Federal regulations.
- Focuses on continued quality improvement.
- Prepares and facilitates community education presentations.
- Ability to maintain statistical information.
- Ability to meet grant goals.
- Perform other duties as assigned, including responding to an emergency event.
Supervision Received
Receives supervision from the assigned supervisor to fit needs of employee (i.e., substance use supervision and/or mental health supervision).
Qualifications
- Bachelor’s degree from an accredited college or university in human services related field.
- Must be able and willing to work evenings.
- Valid driver’s license and automobile insurance are required.
- Prompt and regular attendance.

- Knowledge of community resources.
- Knowledge of evidence-based practices.
- Knowledge of mental health and substance use disorders.
- Ability to work well with treatment team and community members.
- Build and maintain effective working relationships with a diverse client population.
- Ability to work independently and multitask.

- Interacts with the public in a professional manner.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
