Job Description
Job Summary
- Position available for a PT Medical Social Worker in Saco and Portland areas. These patients primarily have been recently hospitalized, have a new diagnosis and/or an exacerbation of a current illness. Their needs consist of interventions such as short term counseling, community resource needs, higher level of care placement, ect. Flexible schedule, you make your own based on your availability and the patient's needs.
Responsibilities and Duties
- Coordinates care planning and delivery with the physician, client and family/caregivers, other healthcare team members and Agency staff to promote optimal client outcome.
- Ensures that changes in client’s clinical status are communicated to other health team members and Agency office staff verbally and in writing in a timely manner.
- Educates clients, their families /caregivers and other staff members as appropriate, in appropriate medical social work modalities and interventions.
- Acquires physician orders for certification and re-certification of home medical social services.
- Provides medical social work modalities and interventions suitable to the client’s clinical status and needs and in accordance with the standard of care for client’s age.
- Provides, adjust or cease medical social work modalities and interventions established upon an ongoing assessment/reassessment of the clients clinical status and in accordance with physician orders.
- Combines assessment data and analyzes data to formulate a clinical finding on client management.
- Delivers medical social work modalities in a setting that optimizes client safety and reduces the possibility of medical/health care errors.
- Recognizes possibility of injuries or threats to life and health in the client’s environment.
- Carries out transfer and discharge planning task in compliance with Agency policy
- Partakes in interdisciplinary team conferences per Agency policy.
- Upholds current knowledge of community resources. Recognizes and utilizes external and internal resources to meet client and/or family needs and to promote optimal client outcomes while services are being provided from the Agency, and for transfer and discharge processes.
