Job Description

GENERAL SUMMARY
The Hospital Violence Intervention Program (HVIP) Medical Social Worker provides case management services for a multi-disciplinary, hospital-based violence intervention program dedicated to reducing re-injury and mortality rates among youth and adults by providing intentionally injured patients with support during their hospital stay and beyond.



ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Under the direction of the HVIP Coordinator, provides hospital and community-based services to patients and families within the CHKD Health System who have experience intentional violent injury.
  • Completes a thorough psychosocial assessment to identify the needs of the patient and family members affected by the incident.
  • Provides immediate crisis intervention support and counseling.
  • Completes mental health evaluations and/or ensures that a referral for mental health support and evaluation is completed.
  • Connects patients and families to appropriate community resources and services to address concerns and needs identified in the assessment.
  • Administers and tracks the use of crisis funds serving patient victims of violent injury.
  • Accompanies victims of violent crime within the hospital, criminal justice system, and other systems as needed to support during appointments and meetings.
  • Provides on-going case management and follow-up services to the patient and family for up to one year following the incident.
  • Plans and facilitates community activities as needed to promote program awareness and assist in reducing community violence.
  • Performs other duties as needed.
LICENSES AND/OR CERTIFICATIONS
  • None required.


MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
  • Master’s Degree in Social Work, Counseling or related licensed eligible field.
  • Course work in case management, advocacy, and family systems preferred.
  • Approximately six to twelve months on-the-job social work experience preferred.
  • Strong knowledge of trauma informed care and practices.
  • Strong public speaking skills.
