About the position:
Under the administrative supervision of the Clinician Manager of Centralized Follow-Up, the nursing supervision of the Director/Assistant Director of Health Services, and clinical guidance (the provision of direction, periodic inspection, and evaluation for the accomplishment of a nursing task or activity) of a designated Clinician(s), provides nursing and medical care to patients seeking reproductive health care in family planning, and AB Health Centers. Supports patients and staff by performing, with accuracy and timeliness, a variety of tasks in the back office or nursing station of the health Center and incorporates those functions into the overall operations of the agency.
This is a .8 FTE union position - 30 hours/weekly Monday - Thursday
Hourly: $30.96-$46.64
This position is fully remote.
What you’ll be doing:
Works within a team environment to render highly professional and technical nursing care to patients.
Provides direct and indirect patient care using the nursing process (assessment, planning, implementation, and evaluation).
Collaborates with multidisciplinary team members to provide appropriate care in accordance with PPGNHAIK policies, protocols, and procedures of the agency and within the scope of his/her RN license.
Under the clinical guidance of a clinician and after appropriate competency verification, performs expanded nursing and medical tasks as assigned.
Performs laboratory tests, collects and processes specimens, takes vital signs, administers medications, and assists clinician and physician during procedures as requested.
Performs triage, via telephone or in person, case management, follow up of referrals and abnormal laboratory results, and chart review.
Triages incoming communications from clients or potential clients per Agency protocol, including but not limited to history assessment, patient information and education, notification of lab results, assessment of symptoms, prescription refills, referrals to same day/urgent/emergent or otherwise appropriate health care resources.
Provides patient education regarding contraception, sexually transmitted disease, including HIV, prevention and treatments, health promotion activities, pregnancy options, and abortion services.
Works remotely with all PPGNHAIK Health Centers on the West side of the affiliate and contributes to the team effort of the agency, thereby ensuring quality care and services for clients. Initiates appropriate follow-up plan, makes requisite number of notifications attempts to notify patient of results and recommended management plan, counsels patients on abnormal results and need for follow-up, and initiates treatment, under Standing Orders, for limited scope of services in accordance with PPGNHAIK Medical Standards and Guidelines.
Exhibits ongoing proficiency in electronic practice management systems, electronic health records, health center policies, procedures, and practices. Documents in medical record and practice management systems; schedules appointments as needed.
Assists in quality assurance tasks via auditing of lab results, referrals, and follow-up plans to ensure completion of follow-up and adherence to established protocols based on PPGNHAIK Medical Standards and Guidelines.
Completes mandatory reports for State Department of Health reportable conditions.
MAJOR RESPONSIBILITIES:
Performs reproductive health and STD services within the scope of her/his license and as delegated by a designated clinician (ARNP) or physician. Services provided include contraceptive method counseling, refilling of contraceptive medications per approved protocol, dispensing medication for treatment of STD’s, collecting of routine specimens, ex. Pap smears and pregnancy tests, treatment of minor conditions, ex. Genital warts, in patients with established diagnoses and defined treatment plans, and administering of oral and parenteral medications. Takes histories, screens patients, and dispenses medication under protocol for new and established method starts and Emergency Contraception visits, under protocol and direction of a clinician or physician.
Performs when required, limited aspects of other back-office duties as assigned.
Maximizes Health Center flow of acute and routine appointments by monitoring status and recognizing delays in client flow, and along with the rest of the team, institutes strategies to improve flow.
Under the clinical guidance of the clinician, assists with follow up activities for abnormal results or physical findings. Activities may include maintenance of follow up systems, notification of patients of results, scheduling of appointments, coordination of referrals to appropriate health care resources, and documentation of compliance with recommended medical regimens.
Performs a variety of tasks in the lab area in accordance with the needs of the team. Provides care in the electronic medical record environment.
In conjunction with the clinician, performs triage according to PPGNHAIK protocol for new, established, and walk-in patients, to ensure timely and appropriate response to patient concerns, medical needs and access to care. Triage may occur in person or via telephone.
In conjunction with, or at the request of the Clinician Manager of Centralized Follow-Up and/or clinician, assists the Education/Training Department in identifying clinical procedures in need of review for existing staff or new employees. Assists with training and mentoring of new or existing staff.
Provides quality and efficient telecommunication or electronic customer service to internal and external customers.
Provides appropriate and medically accurate patient education in support of all services offered by PPGNHAIK.
Provides professional and comprehensive nursing assessment, evaluation, education and advice to patients within PPGNHAIK protocols.
Checks messages and communications via printer, fax, email, EPM task manager, and EMR inbox at least every half hour to triage patient communications and respond to the patient by the end of the day.
Triages calls appropriately based on PPGNHAIK protocols.
Expedites urgent/emergent calls as needed.
Communicates prescriptions to pharmacies to refill prescriptions or to initiate treatment under Standing Order as allowed by protocols.
Documents interventions and education in the EMR of established patients using standard medical guidelines, abbreviations, and EHR documentation standards.
Enters accurate data into the Agency’s Practice Management and EHR systems.
Demonstrates proficiency in the use of PPGNHAIK Practice Management System.
Manages communications in a professional, systematic, and organized manner.
Meets or exceeds Patient Access Center metrics for telecommunication and electronic triage of client contacts.
Maintains knowledge of the PPGNHAIK Medical Standards and Guidelines.
Receive, manage, and direct calls in accordance with call management protocols, including but not limited to:
Completes quality assurance monitoring tools as specified for this department.
Trains or precepts float/per diem RN or Clinician employees who rotate when requested.
Acts as a role model and resource in nursing care and in the development of improved nursing triage systems.
Acts as a medical resource for Patient Access Center staff.
Conducts in-service education for non-licensed Patient Access Center staff, upon request, on birth control methods, STIs, and various reproductive health topics.
Assists the Director of Health Services and/or the Centralized Follow-Up Clinician Manager in developing policy and procedures that promote teamwork, quality of nursing care, and enhance customer service.
Notifies the appropriate contact person when issues or problems arise; follows chain of command.
Makes sound decisions and demonstrates the ability to handle stressful, sensitive, and/or challenging situations appropriately.
Accepts, learns from, and integrates feedback from knowledgeable persons regarding effectiveness of own communications.
Actively and positively strives to implement the goals of the Patient Access Center and Clinical Services.
Completes all responsibilities according to established protocols, policies, and standard practices in the areas of customer service, quality assurance, and regulatory compliance programs such as HIPAA (Health Information Portability and Accountability Act), OSHA (Occupational Safety and Health Administration), and CLIA (Clinical Laboratory Improvement Act).

EDUCATION:
BSN Preferred.
Graduated from accredited RN nursing program
EXPERIENCE:
Minimum of one year nursing experience in outpatient procedures or post-procedural care services.
Three years of experience in Women’s Healthcare with one year experience in Reproductive Healthcare preferred.
Telephone triage experience required.
Experience in a call center environment preferred.
Bilingual English/Spanish skills preferred.
Must be willing to orient in surgical and family planning environment to develop in depth knowledge of health center flow and functionality.
Excellent verbal, written, and interpersonal communication skills.
Ability to perform multiple tasks at once and work in a fast-paced environment.
Ability to be proactive in team development and problem solving.
Ability to work autonomously with a set of protocols.
Must be comfortable working in a paperless environment, including use of the electronic medical record and practice management systems.
Ability to use various telecommunications equipment to facilitate interaction with clients; this includes videoconferencing equipment.
Willing to work flexible hours including evenings and weekends.
Ability to maintain strict confidentiality.
Ability to communicate effectively with clients and co-workers of all backgrounds.
