THE MISSION OF PATHWAYS TO HOUSING
Pathways to Housing PA serves people with complex physical and behavioral health needs who have experienced homelessness. Many of the people we work with have serious, chronic and untreated medical issues that have often been neglected for years and require significant healthcare coordination. To address these needs, Pathways PA established an Integrated Healthcare Program. Our goal is to ensure a person-centered approach to care that emphasizes a welcoming environment, recovery, wellness, trauma-informed care, and the integration of physical and behavioral health care through a harm reduction lens. Pathways, in partnership with Thomas Jefferson University/Department of Family and Community Medicine and Project H.O.M.E.’s Stephen Klein Wellness Center, provides a comprehensive array of low barrier services to improve access to physical and behavioral health care. THE MISSION OF PROJECT HOME The mission of the Project HOME community is to empower adults, children, and families to break the cycle of homelessness and poverty, to alleviate the underlying causes of poverty, and to enable all of us to attain our fullest potential as individuals and as members of the broader society. We strive to create a safe and respectful environment where we support each other in our struggles for self-esteem, recovery, and the confidence to move toward self-actualization. Project HOME achieves its mission through a continuum of services comprised of street outreach, a range of supportive housing, and comprehensive services. We address the root causes of homelessness through neighborhood-based affordable housing, economic development, and environmental enhancement programs, as well as through providing access to employment opportunities; adult and youth education; and health care. Project HOME is committed to social and political advocacy. An integral part of our work is education about the realities of homelessness and poverty and vigorous advocacy on behalf of and with homeless and low-income persons for more just and humane public policies. Project HOME is committed to nurturing a spirit of community among persons from all walks of life, all of whom have a role to play in making this a more just and compassionate society. Job Summary Pathways Medical Assistant has the primary responsibility to perform administrative and clinical procedures to ensure program participants seen at Pathways to Housing receive high quality, efficient care. Essential functions and competencies are as follows:
Essential Duties and ResponsibilitiesPathways Medical Assistant is a key member of the primary care team and instrumental to its success. The primary care team includes the program participant, his or her family or support system, and the participant’s personal clinician (attending physician, resident physician, physician assistant or nurse practitioner and nurse). The primary care team may also include, if warranted, a pharmacist, mental health and/or substance abuse counselor, and/or peer specialist. The Pathways Medical Assistant is a multi-skilled member of the health care team who performs administrative and clinical procedures under the supervision of licensed health care providers.
Preferred Qualifications · CPR and First Aid certification preferred. · At least one year of experience as Medical Assistant. · Spanish language proficiency preferred.

- Determine the preferred mode of communication with patient
- Demonstrate skills required to provide care to patients.
- Assesses needs of the patient, providing proper care to meet those needs.
- Performs various administrative and clinical procedures as required including but not limited to:
- Prepares exam and treatment rooms with necessary instruments
- Show patients to examination rooms and prepare them for examination and treatment
- Maintains clean and neat examination rooms
- Interview patients to obtain medical information and vital signs
- Record patients' medical history, vital statistics and information such as test results in medical records.
- Prepare and administer medications and vaccinations as directed by a nurse practitioner, physician or Physician Assistant
- Collect blood, tissue or other laboratory specimens, log the specimens, and prepare them for testing.
- Prepares and maintains supplies and equipment for treatments, including sterilization
- Assists with scheduling of tests and treatments
- Calling in prescriptions to the pharmacy
- Application of dressings and bandages
- Visual screening
- Carrying out aseptic techniques and following universal precautions and proper OSHA standards
- Provide counseling and instructions regarding common patient problems.
- Scheduling appointments and greeting visitors
- Sorting/Handling mail
- Medical Records filing/updating/record-keeping
- Assisting with submitting medical claim forms patient billing and bookkeeping
- Assists with other clinical procedures and administrative duties as assigned or directed.
- Provides patient counseling services and patient education consistent with directions of the supervising physician.
- Inputs all required data into EMR and other relevant databases.
- Participate in weekly primary care team meetings for case review and discussion of evidence-based chronic disease management, integrated care best practices, etc
- Participate in Quality and Risk Management Committee as designated.
- Develop and maintain activities to achieve Patient Centered Medical Home as required.
- Performs other duties as assigned
