In collaboration with Administrator and Director of Nursing (DON), the MDS Quality and Compliance Coordinator is responsible for collection, submission and reporting of all data and information to meet compliance with established internal and external quality and regulatory requirements for MDS. This position has responsibility for oversite and completion of the MDS process. Provides direction, feedback, and education for all members of the inter-professional team. Monitors and insures team members compliance with established process to support comprehensive patient assessment requirements for MDS completion. Responsible for coordinating the patient care plan and MDS/ Resident Assessment Instrument (RAI) process to assure that desired patient outcomes and services are provided and documented. Duties include formulating or suggesting proactive policy and procedure changes in response to quality and regulatory change impacting MDS.
The position will be working 72 hours every two weeks on the day shift, 8 hour shifts, no weekends. The position is eligible for benefits.Responsibilities/Job Description:

- Transitional Care Unit
- 28 beds
- Serves multiple diagnoses including neuro, ortho, oncology, amputee, spinal cord injury, transplant, etc.
- All adults
- Post-Acute – PT, OT, SLP.
- Approximately 50 therapy and tech staff at the 2 post-acute units
- Highly Collaborative between all disciplines along with MD/Physiatrist, rehab nursing, social work, dietary, etc. with primary goal to get patients home.

- Oversees and is responsible for the day-to-day operations of Care Coordination/Coordinator and MDS ensure support of the mission, goals, and objectives of assigned area(s).
- Demonstrates the ability to implement problem-solving mechanisms.
- Develops and implements standards of practice in patient care.
- Provides feedback, training, education for inter-professional team members
- Provides leadership for MDS quality and compliance improvements measures.
- Provides ongoing assessment and monitoring of unit and patient needs.
- Contributes to management of fiscal resources and cost effectiveness of services provided.
- Demonstrates knowledge of Regulatory Requirements.
- Completes ongoing assessment of policies and procedures related to MDS quality and compliance
- Identifies and initiated quality improvement initiatives to insure
- Coordinates and maintains the patient care plan and MDS/RAI process to assure that desired patient outcomes and services are provided and documented.
- Insures all MDS/RAI are scheduled, completed, and submitted in compliance regulatory requirements.
- Completes Utilization Review
- Ongoing monitoring and feedback to insure integrity of data submitted.
- Maintain a tracking tool for each Medicare patient in relation to requirements.
- Provide and maintain required documentation in the patient record in Epic and PCC.
- Participate in the MDS/QI process.
- Provide MDS/RAI and care plan training and feedback to staff.
- Ongoing assessment of policies and process and continuous quality improvement.
- Identify need and implements necessary changes in response to ongoing changes in required quality reporting and regulatory compliance.
- Bachelor’s Degree in Nursing or Rehabilitation field
- 2 years Rehab, Skilled Nursing Facility or Inpatient Rehab Facility
- Active MN licensure as a Registered Nurse or Licensed/Registered Rehabilitation Clinician
Preferred
- Master’s Degree in Nursing or Rehabilitation field
- 1+ year of MDS experience
