The Manager of Community Health Strategy will partner with internal and external stakeholders to identify local issues, priorities and opportunities for Mercy Health partnership with an emphasis on health equity, economic and community development, community engagement, holistic health and well-being. The Manager is responsible for conducting the Community Health Needs Assessment (CHNA) and Implementation Plan(s), ensuring it meets IRS requirements, industry best practices and is a collaborative endeavor including community partners, community members with special knowledge of underserved populations and internal stakeholders. The Manager will help initiate and mature programs and partnerships aligned to the CHNA and Implementation Plans to maximize the impact of Mercy Health’s local community health response.
Develop effective relationships with community leaders and residents across diverse interests, cultures and socio-economic backgrounds.
Continuously engage local stakeholders and community residents to identify local issues, priorities and opportunities for partnership.
Create, implement and manage progress against Community Health Needs Assessments (CHNA) and Community Health Implementation Plans (CHIP), applying project management skills to ensure the CHNA and Implementation Plan is completed on a timely basis and meets all regulatory and reporting requirements.
Prepare quarterly reports with evaluation metrics to share with Senior leadership.
Develop funding priorities, application and review processes, and ongoing expectations for funded partners.
Manage an active portfolio of funded partners to ensure alignment, outcomes and impact of community health funding.
Partner with the Executive Director of Community Health on strategic initiatives that align to system-wide community health objectives and have the potential to innovate and/or diversify local community health commitments.
Represent Mercy Health on local task forces, community coalitions and advisory groups.
Partner with Director of Community Health Programs and Program Managers to meaningfully incorporate and expand key Market programs.
Initiate and mature relationships with site/hospital leadership to incorporate and align local priorities with regional Community Health strategies.
