Position Summary:
The RN/LPN Case Manager partners closely with the care team to drive clinical outcomes and deliver care across the continuum. The case manager works with in their scope to support clinics by managing and driving targeted interventions for at risk populations. In addition, the case manager will play an integral role in patient education, transition of care visits, and chronic condition management services. - Work collaboratively with providers and the care team to ensure patient adherence to medical plans of care, including all preventive and disease-specific screenings, interventions, and treatment goals including self-management goals
- Interface with high-risk and rising risk patient populations to drive outreach, assist in care planning, and provide motivation and education where necessary.
- Manage and support an assigned population with moderate to complex care needs across the continuum with the purpose of improving health outcomes and resource utilization through a coordinated approach.
- Connect patient to community resources as appropriate
- Executes, and controls the task, phases, and deliverables of population health and clinical initiatives based on the identified project goals and objectives
- Assists patients in navigating the healthcare system while being a steward of the patient health
- Perform duties related to anticoagulation management including collecting specimen, adjusting medication, and providing education.
- Performs general nursing duties such as taking vitals, IV therapy, catheterization, and triage.
- Education:
- Completion of an accredited LPN/RN Program: Required
- Experience:
- 2+ years experience case management: Required
- Certification:
- Current Washington State LPN/RN License: Required
- Current ACLS or BLS: Required
