- Receive and process new patients for admission to our agency using Electronic Medical Record system
- Coordinate care by assigning case manager and clinician(s) to ensure 48-hour admission
- Communicate with various healthcare professionals to provide highest level of care
- Use Health Insurance Portability Accountability Act compliant processes to receive and process orders/faxes for new and current patients
- Perform follow-up calls to patients and caregivers to offer clear and supportive communication
- Perform other various coordination duties to support day to day work flow
- Communicate with Supervisor and notify promptly of adverse findings or unfavorable situations
- Demonstrate proper knowledge of State and federal and payer rules/regulations
- May occasionally be assigned to provide patient care
- Participate in the On Call after hours rotation if needed
- Perform other duties as assigned
- Graduate of an approved Nursing education program
- Current, active license to practice as Licensed Practical Nurse in Tennessee, with at least two years of successful experience in health care (preferably home care), practicing
- Demonstrates excellent oral and written communication skills and problem-solving capabilities
- Knowledge of home health regulations, eligibility and private payer practices
- Demonstrates ability to appropriately prioritize, organize, display assertiveness, flexibility and cooperation in performing job responsibilities
- Must possess and maintain current, valid Tennessee drivers license, reliable automobile transportation, proof of current automobile insurance, current Cardiopulmonary Resuscitation certification by either American Heart Association or Red Cross
