    Education: License:
    Graduate of an accredited practical nursing program. Current NYS license for practical nursing

    Experience: Other:
    1-year geriatric or medical/surgical experience preferred. Ability to spell correctly, produce legible written documentation. Excellent interpersonal skills. Organizational skills and proper body mechanics required.

Essential Physical Demands Of The Job:
Moderate to heavy physical effort with frequent and prolonged standing, walking. Needs to lift supplies/ equipment, residents; pushes/pulls, moves/lifts heavy residents with considerable reaching, stooping, bending, kneeling, crouching. Ability to see peripherally, ability to distinguish and identify different colors.
Essential Mental Demands Of The Job:
Is able to prioritize and respond professionally in a stressful and emotional situation. Interacting with challenging residents, staff and families. Coping with long term illnesses and death and dying.
Manual Dexterity Required:
Requires manual dexterity and mobility to perform detailed resident care skills. Able to write legibly, grasp objects with hands.
II. Essential Job Responsibilities (Weight 40%)
Essential responsibilities:
1 Safe accurate and timely administration of medications and IV fluids with appropriate documentation.
2 Provides treatments and procedures and monitors them for effectiveness. Documents outcomes in medical record.
3 Provides physical psychosocial care to residents in accordance with individual care plans.
4 Identifies the need for RN intervention and seeks appropriate action as needed. Notifies physician of change in condition.
5 Adheres to the resident’s plan of care. Provides input into development, implementation, and evaluation of the plan in conjunction with the RN and/or other disciplines as appropriate.
6 Observes infection control standards while administering meds and treatments and providing direct resident care.
7 Documents resident care legibly, accurately and include pertinent data relative to condition. Monitoring of resident is documented in the medical record and on 24 hr. report sheet.
8 Serves as preceptor for LPNs, CNAs, and students.
9 Assigns, monitors, and evaluates duties to CNA that are within CNA responsibility and gives direction and support to CNA’s to implement each resident’s plan of care. Discusses findings with CNA to improve performance. Acts as a resource for CNA’s and fosters a team approach to tasks and problem solving.
10 Participates in collecting data for quality assurance activities and documents corrective actions for negative findings.
11 Displays analytical and basic problem solving skills. Communicates problems to manager as appropriate.
12 Follows all time and attendance policies.
13 Attends educational programs provided by the facility.
