Resources for Human Development Evergreen program is open and hiring Full-Time LPN in the Bethlehem, PA area. The LPN will assist in the coordination of healthcare services for the program's consumers.

Responsibilities/Duties
- Maintains nursing documentation and medication records
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR
- Assesses residents' health and emotional needs
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc.
Required Experience
- Experience and knowledge of IDD preffered.
Required Education
- Active LPN License, Associated Degree or minimum 60 credit hours towards BS.
Required Qualifications
- Active LPN License
- Valid Driver's License
Program Summary
Evergreen is a community-based program providing comprehensive residential and day services in staffed homes. The program was created to support adults with autism, dual diagnoses of intellectual disabilities and mental illness, developmental disorders, and other neurobiological disabilities, significant mental health issues, as well as medical complications. Emphasis is placed on developing communication skills, building social supports, enhancing daily living skills, supporting the community and social integration, providing trauma-based therapies, and mental health stabilization-with the goal of increased independence and participation within the community.
