Sedalia, MO


On a daily basis, coordinates activities and operations of Individualized Supported Living (ISL) Home or Group Home.

Associates degree (AA or AAS) or combination of education and experience to equal two years. Experience working with individuals with developmental disabilities in community-based services is preferred.

The essential duties and responsibilities include the following. Other duties may be assigned.

  • TASKS
    • Assists individuals served in achieving their goals
      • Learns the personalities, habits, and routines of individuals served and works with them accordingly.
      • Provides encouragement and support to individuals served so they can achieve their desired objectives.
      • Teaches, supports, and models appropriate social skills when working with individuals served.
      • Encourages and empowers individuals served to express their wants, needs, and individual preferences.
      • Provides training as outlined in the program individual’s Individual Support Plan (ISP).
      • Documents progress on ISP objectives daily in a thorough, accurate, and person-centered manner.
    • Monitors and assists with daily living activities of individuals served
      • Administers prescriptions and PRN medication as stated on the physician’s orders.
      • Administers medication within the guidelines of being certified in Level I Medication Aide.
      • Responsible for being aware of medication side effects and adverse reactions. Monitors each individual for such side effects and reactions.
      • Keeps physician informed of changes in behavior or any other factors which may constitute a need for an adjustment in dosage or types of medications.
      • Provides opportunities for community recreational and inclusionary activities. Assists individuals served in planning recreational events and ensures events will promote community involvement and inclusion.
      • Assures the safety and well-being of individuals served and co-workers by following CHS policies and obtaining or administering appropriate medical treatment as needed.
      • Provides transportation as needed, may require the use of personal vehicle.
      • Encourages and teaches individuals served to maintain room and belongings in an orderly manner.
      • Ensures housekeeping tasks are completed while on duty. Also checks housekeeping check list to ensure staff are completing assigned tasks while on duty.
      • Ensures the preparation of well-balanced meals. Ensures individuals served have opportunities for training in daily menu choices, appropriate cooking skills, basic food preparation, and safety in food handling and management.
      • Adopts certification principles as guiding philosophies in delivering services to individuals served.
      • Assists and/or provides appropriate personal care as needed. This requires repeated bending, lifting, stooping, squatting, and/or reaching.
    • Assists individuals served with physician related activities
      • Schedules and makes arrangements for all medical appointments for individuals served.
      • Assures that individuals served have all prescribed medications; and that prescriptions are filled as needed.
      • Collects or assures collection of all specimen ordered by the treating physician.
      • Notifies the Community Living RN and SUPERVISOR if an ordered specimen is not collected within 24 hours of physician’s order.
      • Notifies the treating physician if an ordered specimen is not collected within 48 hours of the order.
    • Completes required documentation accurately
      • Completes the following for each assigned shift:
        • Documentation on ISP objectives – notes progress, changes, concerns, etc.
        • Medication Administration Records – daily and PRN medications
        • Service notes in Setworks at the time of service delivery
        • Staff Duty checklist
      • Completes the Event Report (ER) accurately and completely within the designated timeframe established by policy.
      • Assures records for individuals served are properly organized and up to date.
      • Assures monthly paperwork is completed and submitted to appropriate personnel in a timely manner (monthly reviews, house meetings, emergency drills, etc.)
    • Maintains accurate accounts
      • Attends monthly meeting with Program Support Assistant to balance master accounts and cash accounts.
      • Responsible for turning in grocery and household receipts at the end of every month.
      • Responsible for turning in petty cash receipts to the Program Support Assistant at the end of every month.
      • Assures individuals served’ accounts are not overdrawn.
      • Assures individuals served’ pay portions and phone and cable bills are paid every month.
      • Responsible for following the guidelines set forth in the Financial Procedures while handling individuals served and household money.
      • Assures individuals served reapply for Medicaid services as needed.
      • Monitors individuals served’ resources to ensure Medicaid eligibility.
    • Miscellaneous duties
      • Orders supplies, as needed.
      • Assures maintenance work orders are completed and submitted to appropriate personnel in a timely manner.
      • Responsible for knowledge and implementation of agency and departmental policies and procedures.
      • Presents a positive attitude and demeanor when working with individuals served, co-workers, administrative staff, supervisors and/or parents or guardians.
      • Maintains good communication with the SUPERVISOR; by keeping them informed of developments, concerns, and other pertinent issues regarding individuals served. Also communicates concerns related to the residence.
      • Determines when independent, mature judgment decisions are appropriate and when consultation with supervisor is necessary.
      • Accepts and willingly carries out special assignments or duties, when requested.
      • Respects the rights of all individuals served.
      • Projects positive image of CHS and individuals served to the local community.
      • Assures the confidentiality of information regarding individuals served.
      • Assures expense vouchers are submitted by the assigned deadlines.
      • Works holidays based on program needs or as requested by individuals served.
      • Utilizes the UKG READY system to clock in and out and transfer time correctly. If errors happen employees are required to utilize the “Time Change Request” option in UKG READY to request corrections to their punch or time-off entries.
      • Advise Community Living RN of individuals served’ medical issues.
      • May be assigned to other locations or in the community as needed based on staffing and individuals served’ needs.
      • Carries scheduling pager for a week at a time, on a rotating basis.
      • Staff will understand and appropriately respond to the unique combination of cultural variables through ongoing emphasis on Cultural Competency and training as defined by the CHS Cultural Competency plan.
