Job Description

The Intake Coordinator is responsible for completing the intake and assessment for individuals and families entering the Homeless Assistance Center (HAC). The Intake Coordinator will meet with each client to collect demographic and personal information and will conduct data entry into various database systems. Other responsibilities include orienting clients to the HAC, explaining various forms to clients, assisting medical staff with scheduling client appointments, and providing administrative assistance to the Director. The Intake Coordinator will work collaboratively with the County’s Coordinated Entry System partners, including outreach workers and other community partners who place clients into the HAC. The Intake Coordinator understands that Broward County follows a Housing First model, and that all clients’ experience with homelessness should be rare, brief, and non-recurring.

II. ESSENTIAL DUTIES AND PRIMARY RESPONSIBILITES:
1. Contacts family within two business days of receiving the referral.
2. Conducts initial intake of clients, individuals and families in coordination and guidance from the Director of Clinic and Intake.
3. Completes all required intake forms and enters all required client information into the electronic records systems for new clients entering the HAC.
4. Works collaboratively with case managers and other service providers to assist clients as needed during their move into shelter or stable housing.
5. Documents and maintains up-to-date information in the Homeless Management Information System (HMIS) and in electronic health records within the required timelines.
6. Supports the clinic and the Director in serving clients.
7. Participates in staff meetings and attends trainings as assigned.
8. Manages traffic flow in the clinic and provides general supervision and oversight of clients, ensuring safety and security is maintained at all times.
9. Orients the client to the programs available at the HAC.
10. Manages emergent situations, alerting appropriate staff and/or emergency personnel.
11. Assists in census data collection and enters data into the appropriate systems within the agency’s specified timeframes.
12. Assists with scheduling clinic appointments, provides follow-up with clients, and coordinates transportation.
13. Coordinates client services with other members of the multidisciplinary team.
14. Acts as a liaison between the clinic and other members of the multidisciplinary team, as well as outside resources.
15. Promotes effective communication among clients encouraging and modeling skills in negotiation, judgment, and problem-solving.
16. Demonstrates proficiency in managing and/or de-escalating clients who are in crisis.

Education/Experience: A high school diploma or GED is required. One year experience in a related field. Must have a valid Florida driver’s license and safe driving record. Proficient use of computer software, excellent time management, the ability to effectively resolve and cope with immediate conflict and crisis situations.
