Intake Coordinator

Our care team can be comprised of some or all of the following disciplines: Doctors, Nurses, Certified Nurses Aides, Social Workers, Chaplains, and Volunteers.

Job Description:

The Intake Coordinator is responsible for managing all aspects of the intake process for patients transitioning from One Point Home Health to One Point Hospice.

Duties and Responsibilities:

• Establishing and maintaining positive relationships with customers and referral sources
• Identifies and verifies insurance coverage of hospice services.
• Consults with physicians, nurses, social workers, discharge planners, and other disciplines to establish a coordinated home plan of care.
• Gathers all needed materials to facilitate patient admission.
• Maintaining accurate records for referrals

Basic Qualification:

• Minimum one (1) year experience in Home Health or Hospice agencies.
• High School Diploma or GED is required
• Proficient in using Microsoft Office Tools, Excel experience preferred
• Familiar with Electronic Medical Records System
• Ability to multi-task and work within a team environment.

Job Type: Full-time

Pay: $14.00 - $16.00 per hour
