Job Description:
The Intake Coordinator is responsible for managing all aspects of the intake process for patients transitioning from One Point Home Health to One Point Hospice.
Duties and Responsibilities:
- Establishing and maintaining positive relationships with customers and referral sources
- Identifies and verifies insurance coverage of hospice services.
- Consults with physicians, nurses, social workers, discharge planners, and other disciplines to establish a coordinated home plan of care.
- Gathers all needed materials to facilitate patient admission.
- Maintaining accurate records for referrals
Basic Qualification:
- Minimum one (1) year experience in Home Health or Hospice agencies.
- High School Diploma or GED is required
- Proficient in using Microsoft Office Tools, Excel experience preferred
- Familiar with Electronic Medical Records System
- Ability to multi-task and work within a team environment.
Job Type: Full-time
Pay: $14.00 - $16.00 per hour
