In Office LVN Case Manager-Hospice

We are seeking an In-Office LVN Case Manager to join our Hospice team! You will assist with managing a patient case load, running benefits, placing equipment, ordering medications and much more. Must have hospice experience and working knowledge of EMR program HospiceMD.

RESPONSIBILITIES:

• Consistently monitor case load for patients/families needs
• Triage phone calls
• Monitor and report changes to director of nursing
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

QUALIFICATIONS:

• Previous experience in nursing or other medical fields (1-6 months)
• Familiarity with medical software and equipment (1-6 months)
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Current LVN license

Job Type: Full-time

Pay: $20.00 - $28.00 per hour
