BRIDGES MN HAS ORIENTATION FOR IHS PRACTITIONERS EVERY MONDAY! IHS Practitioners oversee the day-to-day operations of our adult sites, which serves individuals with disabilities. Shift: Monday-Friday 1st Shift

Pay Rate: $15-17/hour based on experience

***MUST BE OPEN TO WORK ANY HOURS/ANY DAY INCLUDING WEEKENDS***

***MUST HAVE YOUR OWN VEHICLE - MILEAGE REIMBURSEMENT***


IHS Job Duties and Responsibilities:

  • For new persons being served with Bridges MN, complete all intake documentation as outlined in ILS worker To-Do checklist and ensure compliance with DHS requirements in terms of content and timing.
  • Provide weekly updates that correlate to ILS service areas, POP, Action items to Supervisor for approval to be sent to team.
  • Build skills to help teach individuals how to navigate their communities and connect with resources for living -knowing their benefits, transportation options, budgeting, etc.
  • Communicate clearly to the person, their team and their Supervisor on a regular basis.
  • Ensure documentation is always complete, up to date and organized.
  • Manage caseload hours and establish routines to best support and provide quality care.
  • Support individuals in their day-to-day needs, build trusted relationships while continually focusing on goals and outcomes to best help them achieve independence.
  • Encourage proper personal hygiene of the person served. Encourage and help maintain a clean living space.

IHS Requirements:

  • Be able to manage a caseload of resident's needs for IHS services.
  • Acceptable Driving History and dependable transportation.
  • Ability to pass a background study.
  • Develop outcomes with the person using person-centered tools.
  • Provide support and/or training to people who live in their own home or their family’s home in the following community living service categories: community participation, health, safety, and wellness, household management, and adaptive skills.
  • Outline and encourage personal cares as well as mental health updates.
  • Build rapport with persons served and encouraging positive decision-making.
  • To be successful in this position, the applicant must be able to constructively communicate with residents, as well as be diligent and persistent. Some residents will refuse services and we are looking for someone who can change that around so we can help and improve the lives of those we serve.
