Job Description

The AIDS Center of Queens County, Inc. (ACQC) has a strong commitment to providing HIV/AIDS prevention,care, and advocacy for individuals throughout NYC. A leading provider of services since 1986 for those who are HIV+ and at risk, ACQC seeks to educate,empower and improve the lives of those we serve.

The Housing Specialist/Case Manager will provide housing placement services, manage a client caseload and coordinate services in order to move clients toward self-sufficiency and optimum health.

Responsibilities

  • Provide advocacy and referral services to ensure that clients receive needed benefits, entitlements, and services such as medical and mental health care.
  • Assess and identify clients’ needs in order to develop service plans for each client.
  • Maintain client case records, both hard copies and computer entries as applicable, containing all appropriate documentation, correspondences, narratives, and forms, which reflect ongoing implementation of service plans.
  • Develop and maintain professional relationships with clients in order to monitor physical and emotional capacity for independent living and the need for additional services.
  • Facilitate Independent Living and Housing Readiness Workshops together with other staff.
  • Provide individual independent living skills and housing readiness counseling for program clients.
  • Identify and secure apartments for client placement that are in well-kept buildings and are in conformance with existing city/state housing laws and HASA standards.
  • Assist with intake and admission processes.
  • Follow COVID guidelines to keep staff and clients safe while in the office.

Required Qualifications

  • Bachelor’s Degree with related experience or High School Diploma with 3 years’ related experience.
  • Sensitivity to cultural, ethnic, and gender orientation differences.
  • Bilingual ( Spanish Speaking)

The Housing Specialist position is a full-time, non-exempt position, and is based in Jamaica,Queens near the E and F trains.

The ideal candidate for the Housing Specialist position will be a hard working team player who has a passion for human services and helping individuals find stable housing.
