Job Description

Assists in coordination of in-home support services to meet the needs of Hospice patients and their families. Performs introductory visits to patients and families. Will be responsible for providing nursing services consistent with high standards of performance and conduct with currently available knowledge and nursing skills acquired through completion of practical nursing program. Makes ongoing evaluation of the patient and family’s needs, including need for change in medical management to control symptoms and maintain comfort. Applies assessment and crisis intervention skills. Coordinates resources available to meet patient and family’s needs. Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines. Assists with and completes other tasks as requested.


Required Skills
  • Licensed in the State of Montana, good standing.
  • Must have knowledge of the application of diagnostic and crisis intervention skills.
  • Must have the ability to respond calmly and effectively in stressful situations; assess patient needs; reassess continuing patient needs; communicate effectively with other health care providers.
  • Strong, effective written and verbal communication and interpersonal skills.
  • Must possess strong organizational and time management skills.

Physical Requirements

  • Ability to move independently or with reasonable accommodation within the facility and community.
  • Applicant must be physically and mentally able to perform all requirements of the job.
  • Current driver’s license and reliable transportation.
  • Applicant must pass a background check.


Required Experience
  • 3-5 years
  • Three to five years experience working with elderly or terminally ill preferred.
  • Associate's degree
  • Currently licensed as practical nurse.
  • Current CPR certification.
