What you will do
Responsible for overall organization and financial management of the agency, direction of agency operations and functions according to approved policies, procedures and standards. Ensures the delivery of quality care to patients, enhancement of business development, and continuous improvement of agency efficiency and fiscal success. May fulfill role as Agency Administrator or Supervising Nurse for state licensure purposes.
- Be available to the branch sites by phone, fax or email to maintain ongoing liaison with the branch sites.
- In collaboration with the Branch Directors, assists with the implementation and monitoring of the Performance Improvement Program, developing Corrective Action Plans as appropriate.
- As a member of the PI committee, meets quarterly with the Branch Directors to review PI data for all sites, summarize and document the findings on the parent office PI committee minutes.
- Provides educational presentations to the Directors and staff as appropriate for any trends identified in the Performance Improvement quarterly reviews.
- In collaboration with Branch Directors, plans and participates in periodic management meetings to assure coordination of services, provision of quality care delivered by branch sites, staffing issues, staff competencies and onsite supervisory visits are being conducted according to agency policy, Performance Improvement and implementation of consistent processes within branch offices. The meetings will be documented and maintained in a file or notebook by the DOO in the parent office.
- In collaboration with Branch Directors, assists in planning, organizing, and conducting the Professional Advisory Group meetings.
- Periodic on site visits to provide oversight, guidance and direction to the Branch Directors to ensure compliance with all regulatory bodies.
- In coordination with the Branch Directors, assure that all visiting staff are deemed competent by conducting on hire and annual skills competencies to ensure staff is competent and are able to provide appropriate, adequate, and effective patient care.
- Leads and facilitates the development of the Overall Agency Evaluation which includes data from all branch sites.
- Leads and facilitates agency leadership meetings in preparation for ACHC Accreditation, and in collaboration with the Branch Directors, plans and implements action plans and educational activities to assure agency readiness.
- Monitors the home health compare measures when published, plans and develops action plans in collaboration with the branch directors to improve the measures(Acute Care Hospitalization and Emergent Care) that are above the State Average, and as indicated any of the 11 measures that are below the State average. Implementing and providing education to clinical staff on best practices to improve the Target Measure/Outcomes.
Qualifications
Required
- Registered nurse OR a health care professional licensed to practice in the state with supervisory or management experience in a health care setting OR an individual with education, training and experience in health service administration with management experience in a health care setting.
- Demonstrates knowledge of Federal, state, local and ACHC regulations.
- Demonstrates organizational and communication skills which would enable effective interaction with agency staff, patients, families, physicians, health care providers, and the community.
- Minimum two (2) years experience in hospice setting preferred.
- Possesses current driver's license, agency required liability insurance, and reliable automobile transportation.
- Current CPR certification is required if the DOO is a licensed clinician.
- Knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements.
- Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers.
