Job Description
Status: Full-Time, 64 hours per pay period
Shift: Day– weekend & holiday rotation
Pay Method: hourly/non exempt

Position Summary: The Hospice Care Aide is responsible to provide mission-centered care to patients in homes or contracted long term care facilities following the patient’s individualized Plan of Care, under the direction of the patient’s nurse and/or physician and in accordance with State and Federal Regulations and St. Anthony’s philosophy, standards and policies.
Essential Job Functions:
- Adheres strictly to the patient’s individualized Home Care Aide Care Plan. Reports discrepancies in the care plan to the patient’s primary nurse.
- Performs complete or partial bath in bed or shower using safe techniques/appropriate equipment while respecting the patient’s privacy and individual preferences. Shaves patient as needed.
- Performs oral hygiene using toothbrush and toothpaste, toothette or denture cream as directed by the hospice nurse.
- Provides appropriate hair, nail and foot care.
- Educates caregiver on skin breakdown prevention by demonstrating how to properly turn a patient and/or use pressure-reliving devices.
- Assists with dressing and undressing based on the patient’s needs and abilities.
- Assists the patient with drinking and/or eating – meals or snacks – according to the patient’s care plan and instruction from the hospice nurse. Places the patient in the proper position before attempting to give any food or drink.
- Responds in a timely manner to patient’s elimination needs including incontinence care, assistance with ambulation to/from the bathroom or BSC, assistance with bedpan and/or urinal, assistance with colostomy or catheter care.
- Safely provides assistance with ambulation and assistive devices, range of motion exercises, patient transfers and repositioning according to the patient’s care plan.
- Ensures that the room the patient resides in is generally tidy by removing dirty linens/towels/clothing, emptying trash cans or changing bed linens, if requested.
- Assists with patient exams and treatments under the direct supervision of the hospice nurse.
- Provides activities to promote the patient’s physical, mental, social and emotional well-being.
- Monitors and reports changes in the patient’s condition to the hospice nurse in a timely manner.
- Accurately completes clinical documentation that supports daily tasks and the patient’s individualized care plan in a timely and acceptable manner. In Continuous Care cases, this documentation must be recorded hourly, at a minimum, and be reflective of the patient’s status, the patient’s surroundings, the caregiver’s response and the specific things the HCA provided to either the patient or caregiver.
- Communicates with patients, caregivers and team members to share and elicit information related to the patient’s care and treatment.
- Participates in staff meetings, in-services, special team meetings, etc. as assigned by their supervisor.
- Checks and responds to emails on a regular and acceptable basis.
- Adheres to St. Anthony’s Hospice policies pertaining to dress code, conduct and confidentiality.
- Complies with St. Anthony’s Essential Team Values.
- Completes other duties as assigned that may be deemed appropriate.
Professional Qualifications:
- Must be a Certified Nurse Assistant in the state of Kentucky.
- Must be familiar with the use of computers including internet navigation and email use.
- Must demonstrate commitment to St. Anthony’s Hospice mission-centered concept of care.
- Must meet all legal and regulatory requirements of both federal and state governments as well as Medicare Hospice Conditions of Participation and JCAHO.
Physical Qualifications:
- Must be able to operate a motor vehicle to drive to and from patient homes or other assigned St. Anthony’s worksites.
- Must be able to remain in a stationary position to complete patient documentation.
- Must be able to frequently move throughout the patient’s home or contracted long term care facility to attend to the needs of the patient and caregiver.
- Must be able to communicate verbally, via email or in writing with patients, caregivers, visitors, coworkers and physicians.
- If working in the Nursing Home setting: Must be able to lift 50 pounds and push/pull 50 pounds, 8 – 12 times per day to perform essential job functions of bathing, transferring and providing general patient care.
- If working in the Home Care setting: Must be able to lift 65 pounds and push/pull 75 pounds, 8 – 12 times per day to perform essential job functions of bathing, transferring and providing general patient care.
- Must wear appropriate Personal Protective Equipment when there is a risk for exposure to blood, body fluids, cleaning chemicals and other potentially harmful substances while caring for patients or completing housekeeping duties.
- Must be able to use appropriate medical equipment and devices needed to care for and treat patients.
