Wesley seeks a full-time Hospice Aide (Certified Nursing Assistant) to work for its Home and Health Services Department.
JOB SUMMARY
The Hospice Aide is responsible to provide direct care to hospice patients in their home to improve quality of life and promote comfort according to the Hospice plan of care. The Hospice Aide performs tasks required (for example, bathing a client) to help maintain and restore the physical, social and emotional health of patients under the direct supervision of the Hospice Registered Nurse. Additionally, the Hospice Aide provides care and support to patients and families during times of crisis and end of life.
ESSENTIAL FUNCTIONS
- Works within the scope of practice for Hospice Aide under the supervision of the registered nurse. Able to follow instructions provided verbally, in writing, and in the EMR mobile device (iPad).
- Follows the HA plan of Care to deliver quality patient care, including:
Bathing including in bed or out/dressing/nail care/skin care/assist with elimination/nutritional needs/mobility transfers/ Homemaking-maintain a clean safe environment /vital signs, all per care plan. - Observes changes in patient conditions, documenting and report when indicated. Recognizing emergencies and respond appropriately.
- Promotes teamwork by participating actively and appropriately as a member of the interdisciplinary team ( IDT), with monitoring physical status, strengths, coping mechanisms, personal goals, significant relations, home environment and financial status.
- Demonstrates proficiency in use of agency chosen software, EMR, iPad documentation and other systems and technology as required. Documentation is complete, legible and clearly reflects care given and is completed by the end of the assigned shift or visit. Demonstrates ability to effectively communicate verbally and in writing/data entry in English.
- Functions in a financially responsible manner by meeting agency guidelines for number of shifts worked and participation in weekend/holiday rotation. Meets performance standards established according to the Wesley Homes mission statement.
- Works in diverse social and economic situations without imposing own values on patient and family. Maintains appropriate boundaries.
- Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service. Represents WH to the community in a positive manner. Provides information to community members on end of life issues, hospice and palliative care.
- Adheres to Agency, State and Federal policies and procedures, laws and regulations which are relevant to job responsibilities. Demonstrates understanding of Medicare Hospice regulations including HIPAA Privacy Standards and related Agency policies.
QUALIFICATIONS/REQUIREMENTS
Education Background
- Minimum of a high school diploma or equivalent.
Licensure, Registration, Certification
- Nursing Assistant-Certified (NAC) with the state of Washington, WADL.
- Successful completion of Home Health/Hospice Aide competency evaluation requirements required to perform duties according to Medicare Conditions of Participation.
- CPR and First Aid certification or the ability to obtain certification before starting employment. Certification must be kept active during the course of employment.
- Must have reliable transportation, current State driver’s license, and valid vehicle insurance.
Work Experience
- Minimum of one year of NAC/HHA/HA experience required; hospice/home health experience preferred.
INFECTIOUS RISK CATEGORY
HIGH RISK. Likely to have contact with patient's blood or other body fluids May require protective equipment to be worn during some aspects of direct patient care.
SOME (COVID-19) RISK: Possible occupational exposure to residents or clients with COVID-19. May require personal protective equipment (PPE) to be worn during some aspects or seasons of job.
