Home Health RN Or LPN- 5 To 20 Hours Per Week

Giving Home Health Care is a patient centered company delivering in home services to aid our patients in being able to live safely in the comfort of their homes.

We are currently seeking 1- 2's RN's / LPNs to work with patients in the Pueblo. Each shift is 5 hours and there are 4 shifts per week. Days are flexible as they are set among the team with the patient and family.

This is a independant contractor position giving you the flexability to set your schedule with the patient and other qualified nurses.

This is a great way to join a team of dedicated professionals while allowing a patient to stay in their home while making some extra guaranteed income.

Duties Include

Complete nursing assessment

• Complete full body assessment with special focus on the patients covered diagnosis
• Monitoring for changes in condition such as weight loss/gain, self-care abilities, indicators of disease progression
• Medication administration
• Take Vitals
• Assess for indicators of infections
• Educate and assist patient to complete ADL's as needed
• Provide education and training to patient, family and home health aide on disease process
• Communicate with outside providers PRN to ensure there is a continuity of care
• Assisting a family in being able to keep their loved one in the home
• Other duties based on patient care plan

Pay: $37.00 - $45.00 per hour
