POSITION TITLE: RN - Home Health
POSITION SUMMARY:
Responsibility and accountability for a group of patients for a designated time frame; providing care through the use of professional RN skills, the nursing process, and other interdisciplinary team members. Duties are performed within the patient’s home or place of residence and within the Home Health Department.
ESSENTIAL JOB FUNCTIONS:
- Will complete 5 visits per 8-hour shift or 6 visits in a 10-hour shift, unless otherwise directed by the administrator or clinical coordinator.
- Performs accurate OASIS data collection as integrated into the comprehensive assessment at the admission visit, recertification visit, transfer to skilled care, resumption of care, and discharge.
- Coordinates and manages the patient’s home care.
- Modifies the plan of care based on changes in the patient’s condition and physicians orders.
- Plans for discharge needs: addresses patient self-management, caregiver needs, follow-up with health care providers, medications at discharge, and additional services as needed.
- Will perform duties of charge nurse when assigned on weekends and holidays , will take call after scheduled hours to ensure coverage 7 days a week
- Schedules visits in compliance with physicians orders and insurance authorizations.
- Makes appropriate referrals to other SLHH disciplines and to community resources .
- Reports suspected or observed abuse to appropriate state agencies and the clinical coordinator/administrator of SLHH
- Communicates with the biller to assure pre-authorizations are obtained.
- Informs the Unit Manager of unexpected problems, unsafe conditions, and barriers to the provision of home care.
- Participates in interviewing and evaluating prospective new RN staff.
- Demonstrates flexibility in staffing to meet patient needs.
- Perform preceptor duties for new hires and students.

Minimum Requirements
QUALIFICATIONS:
REQUIRED:
Valid driver’s license and private vehicle or proof of reliable transportation with good driving record; and Proof of current auto liability insurance for self or driver. Excellent communication skills; and 2 years recent nursing experience with 1 year experience in a medical surgical or critical care setting. (Exception to previous nursing experience is the successful completion of a work study program approved by Sky Lakes Medical Center and Oregon Institute of Technology.) Employees hired under this special program will be hired supplemental and will not be eligible for a status position until the 90 day probation period is completed successfully.
LIC/REG/CERT:
Valid Oregon RN licensure.
NON-REG CERT:
AHA BLS certification required by end of month from 60 days of hire and maintained; Valid driver's license.
TYPICAL PHYSICAL/MENTAL DEMANDS:
Medium to high physical and mental demand capabilities required. Must be able to physically maneuver through unpredictable and unalterable physical spaces and to be able to climb more than two flights of stairs. Must be able to work independently with patients within individual private homes and residences and with coworkers and to provide service to patients of all ages. Must be able to prioritize patient care needs and be able to give directions to other care providers. Must be able to lift up to 50 lbs.
PAY RANGE: $38.42 - $56.57
