Home Health RN Case Manager-Weekend

Envision Home Health & Hospice is looking for a Weekend Part Time to Full-Time Registered Nurse who is passionate about providing quality care to patients and families enrolled in our Home Health programs within King County including Tacoma and surrounding areas. This position can be 16 hrs to 32hrs. Guaranteed salary plus bonus available!

Responsibilities:

• Develops and follows plan of care appropriate and specific to patient upon admission.
• Admits new patients as needed
• Provides those services requiring substantial specialized nursing skills.
  
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Informs personnel of changes in the condition and needs of the patient.
  
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Participates in in-service programs.
• Understands and adheres to established Agency policies and procedures.
• Processes orders and notifies physician of patient needs and changes in condition.
• Participates in peer review and performance improvement as assigned.
  
• Gives total patient care as needed.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames

Qualifications:

• Current RN license to practice in the state of state you are applying
• Current driver's license, reliable transportation and auto insurance
• Maintain current CPR certification
• Minimum of at least one year nursing experience with at least one year an acute care setting; home health experience preferred
• Self-directed and able to work with minimal supervision
• Demonstrates excellent observation, problem-solving, verbal and written communication skills, nursing skills per competency checklist
• Shows ability to organize and prioritize workload independently
• Wound care experience preferred
