Job Description

    Home Health Registered Nurse (RN) Case Manager



    Tarrant County, Texas



    For over 23 years, this financially stable, locally owned and operated home health agency has been providing exceptional, compassionate and personal care to patients and their families throughout the DFW Metroplex.



    We are seeking a talented Home Health Registered Nurse for 40 hours per week .



    Job Summary:



    As the RN case manager, you will plan, organize, and direct home health care for your patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. You will be the central point for the patients care plan and will help lead and mentor our Home Health Aides. Our case managers will provide care through our service area.



    Qualifications:




  • A passion for home health care and the ability to build relationships with both patients and their family


  • Experience providing home-based care (required)


  • Current licensure and CPR certification


  • The ability to work efficiently with little direct supervision


  • A valid driver's license and an insured automobile that is in good working order


  • Graduation from an accredited school of nursing

  • Managing an average caseload of 10 patients

    • Compensation:




  • $70,000 - 80,000/annually


  • Mileage reimbursement at $0.40 per/mile
