Our Home Health team provides services to support clients and their caregivers in their homes. Our services range from care for chronic illnesses such as diabetes, cardiac issues, and kidney problems to shorter-term care following an accident or surgery. We also provide care when clients finish a stay in short-term rehabilitation at one of our communities, services from WesleyLife at Home can help keep them moving in the right direction! This person would work up to 40 hours a week and will work during the days of Monday - Friday 8:00 AM-4:30 PM with rotating weekends as our Home Health Home Care Aide.

We are looking to cover the Greenfield/Stuart/Adel area and the East Des Moines area.
The minimum pay rate for this position is $14.50/hour for non-certified aides and $16.30/hr for a CNA. Starting pay rates will be based on years of experience.

As a Home Health, Home Care Aide, you will provide the support that will allow our clients to live as independently as possible. Your principal responsibilities will include:
- Traveling to complete visits for each assignment at the pre-scheduled time and location.
- Following each client’s individualized care plan, schedule, and assignment as directed, maintaining a consistent schedule and work environment.
- Providing clients the attention needed to create a sanitary and orderly environment
- Assisting with personal care including bathing, nail and skincare, oral hygiene, and toileting
- Assisting clients with light housekeeping, home management, and safety-related functions
- Participating in staff meetings
- Requires travel

This position will work with clients of WesleyLife Home Health in central Iowa. This role will be required to take visits in the following counties: Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair and Poweshiek.

Experience Required- Valid driver’s license, reliable transportation, valid insurance, and clean driving record
- Ability to pass a pre-hire background check, pre-employment physical and drug screen
- 1 - 3 years: Previous direct care experience
- CNA certification preferred
- High School or better
- Driver's License
- Certified Nursing Asst
