Job Description

We offer innovative primary care across all sites of our primary care led organization. We're seeking the right candidate to join us for a full-time, onsite position at Vanguard Medical Group. This position primarily sits at the Verona location, and will require travel to other VMG sites and home visits with patients. The Social Worker uses culturally competent skills to assess individual patient needs and refers to appropriate professional staff when necessary. S/He identifies barriers and uses interventions to advocate for patients, and has experience/training in medical and clinical environments.

Key Responsibilities:

  • Follows-up on referrals from PCPs and Care Coordination regarding Social Determinants of Health including issues with food insecurity, transportation, housing, and safety in the home, among others;
  • Manages referrals regarding abuse, neglect, domestic violence, and crisis intervention. Performs educational sessions for health care team related to abuse/neglect issues and reporting of same;
  • Assist patients and families in applying for Medicaid, SSI and other income- and disability-based financial programs;
  • Manages data collection and information gathering on patient care issues to better collaborate with Population Health and Care Coordination teams;
  • Attends and participates in regular staff meetings, and on practice-wide committees to analyze and respond to issues significant to social work services and to identify opportunities for improvement;
  • Provides timely, accurate and detailed documentation in managing patients;
  • Other responsibilities as they arise in conjunction with requirements for value-based contracts.

Job Essentials:

  • Excellent communication skills (written and verbal) and the ability to build trusting relationships with patients
  • Working knowledge of local community and regional resources
  • Creative and resourceful problem-solver; excellent time management skills

Position Requirements:

  • Master’s degree in Social Work
  • Current licensure in the State of New Jersey
  • 5 years of current working experience in a patient-facing environment preferred
  • Fluid skills in Word, Excel, and electronic health records
  • Bilingual speakers a plus
