Job Description

Shift: Full time, Days


Reporting and accountable to the Manager of Health Information Management Operations ; facilitates the processing and maintenance of patient medical records utilizing multiple computer based record systems; responds to patient, physician and other inquiries related to medical records.

1. Retrieves records for scanning from drop-off shelf Completes scanning and indexing with the highest quality on a daily basis.

2. Performs prepping and scanning of records. Removes staples, paper clips, bands, etc. and scans documents. Identifies pages that did not scan well and re-scans documents accordingly.

3. Routes results to the appropriate physicians as they get scanned to the medical record.

4. Able to identify correct referral orders in Epic and closes them appropriately when scanning the report to the patient’s chart.

5. Able to identify appropriate NCH forms and documents to be scanned into the legal record and reports all unidentifiable forms to the Manager of HIM.

6. Works on Epic work queues for failed interfaced reports. Validates correct patient and demographics and corrects errors. Sends report back into Epic, ensuring it files to the correct patient under the correct report type.

7. Sets up new physicians in the Nuance system as needed

8. Answers department calls with excellent customer service. Routes callers appropriately.

9. Performs Release of Information for other healthcare facilities and assists patients on retrieving copies of their medical records. Routes the legal medical record to the appropriate requester via Right Fax.

10. Responds to requests for records from various outside sources in a timely manner ensuring appropriate approvals are in place, requested records are available, and that all records release policies are followed.
11. Maintains Right Fax work queues. Resends failed routing or researches the reason for failures. Maintains all scanning equipment on a daily basis and able to troubleshoot equipment issues.

12. Completes monthly chart audits to determine the timeliness of H&P and Operative reports

13. Knowledgeable with all HIPAA requirements and medical records regulations

14. Works on special projects as requested by HIM Management.

15. Adheres to all Northwest Community Hospital standards, policies, and procedures.


Qualifications


1. High School diploma required.

2. One year medical record experience in a healthcare environment with knowledge of and experience with health information computer systems preferred.

3. Knowledge of Medical Terminology preferred.

4. The analytical skills necessary to resolve errors effectively and to identify opportunities for improvement.

5. The organizational skills necessary to prioritize and complete multiple detail oriented tasks simultaneously.

6. Ability to work efficiently with minimal supervision in a fast paced environment while exercising attention to detail and independent judgment within stated guidelines

7. The verbal communication skills necessary to present information in a clear, concise manner.

8. The customer service skills necessary to interact positively with patients, physicians, staff and others that request information from the department.

PHYSICAL REQUIREMENTS:
1. Requires moderate physical effort. May require continuous standing or walking. May require frequent lifting or moving of average weight material, occasional moving or lifting of heavy weight material.
