Providence is calling a Grief Support Counselor (1.0 FTE, Days) to work in Spiritual Care Seattle at SCS Hospice in Tukwila, Washington.

The Grief Support Services Counselor provides Grief Support Services to hospice patients, families, and the community. General duties include conducting bereavement assessments, providing anticipatory and bereavement counseling to individuals and groups, and providing education and training in the field of loss and grief.

In this position you will:

  • Coordinate the day-to-day provision of bereavement services for hospice and community clients in association with the Grief Support Services Manager & the Grief Support Services Assistant.
  • Consult with patient care staff regarding requests related to potentially high risk bereavement and complicated grief.
  • Assist in the completion of the bereavement assessment and develop appropriate bereavement care plans for hospice and community clients.
  • Initiate phone contact with grieving family/friends to present services, offer support and resources.
  • Provide one-on-one grief counseling to agency and community clients.
  • Screen for and facilitates grief support group meetings as offered by program.
  • Coordinate with Grief Support Services Assistant regarding client needs when Grief Support Services Manager is out of the office.
  • Participate in Hospice case conferences.
  • Provide and monitor care by managing assigned caseload.
  • Schedule visits for optimal care, case management and team effectiveness.
  • Document care provided reflective of client goals and within timelines established by program.
  • Inform manager of changes in caseload acuity or available openings affecting staffing.
  • Contribute to overall team function through effective time management and communication; utilize critical clinical thinking and problem-solving skills to address team issues.
  • Participate in Quality Improvement activities, identify related issues and implement recommended changes as appropriate.

Qualifications:

Required qualifications for this position include:

  • Master's Degree of counseling, psychology, social work or theological studies/pastoral ministry.
  • 3 years of post master's work experience. Prior internship at PHOS or other relevant social services-related role counts towards the 3 years of post master’s work.
  • 1 year of experience in health care setting.
  • 1 year of experience providing grief counseling to individuals and/or families.
  • Current Counseling Certification or License through Washington State Department of Health.
  • Current Washington State Driver's license.
  • Automobile insurance.
  • Current Healthcare Provider CPR certification.

Preferred qualifications for this position include:

  • 2 years of experience in health care setting.
