The Nathaniel Clinic is an Article 31 clinic regulated by Office of Mental Health and located in the heart of Central Harlem. The mental health clinic serves individuals aged 13 and above and adults with a specialization in those who are involved in the legal system. The Nathaniel Clinic is part of the Behavioral Health division which consists of a continuum of services for justice-involved individuals with serious mental illness, including outpatient clinic, health home care management (HHCM), assertive community treatment (ACT), and intensive mobile treatment (IMT).

Job Description
  • Educate clients about the benefits of active Medicaid health insurance
  • Encourage clients to enroll in the Medicaid plan of their choice and assist clients in completing Medicaid applications
  • Help clients obtain personal documents necessary to activate Medicaid (e.g., state-issued identification, birth certificate, or social security card)
  • Escort clients to government offices to activate Medicaid
  • Obtain necessary consents/releases to assist clients with entitlements issues
  • Document entitlements services in the electronic medical record in an accurate, thorough, and timely manner, as per program guidelines
  • Track Medicaid-eligibility weekly, as well as upcoming changes in insurance status, and recertification dates
  • Provide care management services in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner
  • Plan and coordinate care management needs, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services
  • Maintain engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations, or have a history of homelessness or involvement in the criminal justice system
  • Conduct screening and assessment of high-risk needs, such as substance use, psychiatric symptoms, harm to self/others, abuse/neglect, or domestic violence
  • Conduct monitoring and follow-up services to promote continued engagement in referred rehabilitative and treatment services
  • Provide 24-hour crisis intervention on-call services on a rotating basis

Qualifications
  • Bachelor’s Degree with a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, or other human services field
  • Candidate must have comprehensive and detailed understanding of federal, state, and local benefits and entitlements systems, especially related to Medicaid insurance. The ideal candidate will have 3+ years experience doing similar work
  • Candidate should have excellent interpersonal, communication, engagement, motivational interviewing, and advocacy skills.
  • Individuals with lived experience in obtaining Medicaid coverage and related entitlements are strongly encouraged to apply
  • Bilingual Spanish-speaking preferred
