Presentation Rehabilitation and Skilled Care Center is committed to providing the seniors of Brighton and the surrounding Boston communities with the highest level of care in a quality setting they deserve Presentation Rehabilitation and Skilled Care Center is looking for a Director of Social Work to assist the residents and their families with the psychosocial, emotional issues related to illness or aging.

Candidates must have Long Term Care experience. Assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social work department. Must perform responsibilities in accordance with current Federal, State and local standards/guidelines and regulations.

Qualifications:

· Must possess a Bachelor’s degree, preferably in Social Work and a current) Massachusetts State license to practice Social Work (a must).MUST HAVE A CLINICAL LICSW or LCSW

· Ability to assess needs from admission to discharge, develop treatment plans and document interventions/information in medical records.

· Coordinate appropriate discharge planning options

Job Type: Full-time

Pay: From $85,000.00 per year
