Diabetes Educator

Vanguard Medical Group is looking to hire a full time RDN (Registered Dietitian Nutritionist) who is a CDCES (Certified Diabetes Care and Education Specialist) provider to join our team. The RDN/CDCES is an important part of the Vanguard team serving as an expert resource in Diabetes Self-Management Training and the related nutrition aspects. This position will allow for the ability to plan, develop, and implement new programs/offerings, as well as provide DSMT and Medical Nutrition Therapy on an individual basis.

Job Responsibilities:

• Provide Diabetes Self-Management Training (DSMT) to patients based upon AADE 7 standards of care
• Provide Medical Nutrition Therapy to patients per physicians? orders
• Manage the diabetes program in compliance with the ADCES (Association of Diabetes care and education specialist) guidelines
• Complete individualized nutrition assessments and development of personalized goals/treatment plan
• Collaborate with physicians on patients? diagnosis and risk factors to further promote delivery of individualized care
• Provide lifestyle change guidance per CDC DPP (Diabetes Prevention Program) standards
• Educate patients and families on blood glucose monitoring, blood glucose pattern management, insulin administration and safety guidelines, and insulin pump/continuous glucose monitoring system training
• Provide patients with resources for obtaining diabetes supplies as needed
• Assist with research projects including data collection, patient recruitment, and other duties as needed

• Bachelor’s degree in nutrition or health related field (Required)
• Master’s degree (Preferred)
• Registered Dietitian (Required)
• Certified Diabetes Care and Education Specialist (CDCES) (Required)
• 2 to 3 years practice experience providing DSMT and MNT
