CT STRONG-PEER OUTREACH & SUPPORT SPECIALIST

Summary:

Team member of CT Strong, focusing on engaging teens/young adults in the wraparound process, and supporting young people by connecting them with needed services.

Essential Duties & Responsibilities:

• Engage young people in supporting their involvement in treatment, support services and advocacy
• Reconnect with individuals who disengage from services.
• Conduct peer support groups
• Participates in wraparound team meetings as required
• Assists in the documentation and maintenance of client case records in the ECR
• Participates in programmatic and team meetings as directed by supervisor
• Perform any other department or agency-related duties or special projects as directed by supervisor.
• May transport clients (Valid license and reliable transportation)

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

Education &/Or Experience:

High School diploma with lived experience as the family member of adolescent consumer in the mental health system. Recovery Support Specialist Certificate. required. Computer literacy a must.
