Job Description
The Critical Documents Associate (CDA) is responsible to assist clinic providers with the completion of forms, prior authorizations, medication refills, telephone encounters, and other documents as needed. The CDA approaches his/her tasks in a team-based care fashion that supports patients and their families in self-management, self-efficacy, and behavior change. Tasks can be completed from home or from the clinic, although at times being present in the clinic may be required.
This position will travel between the Estes Park and Ft Collins clinics to collect documents and review tasks each site needs.
We offer:
- Starting hourly pay range is $17.00 to $19.55 per hr.
- Mileage Reimbursement
- Monday to Friday – Available 8 am to 5 pm

Specific Duties:
- Processes critical documents following SALUD protocols, including, but not necessarily limited to:
- Medication refills
- Prior authorizations
- Telephone encounters
- Various patient care forms
- Others as assigned
- Follows procedures for incoming and outgoing forms using the electronic health records system
- Delivers and retrieves patient records throughout the system as needed
- Maintains a tracking/documentation system to ensure completion of forms and filing of documents in the designated locations
- Reviews each form to determine the accuracy of patient information, completion
- Prepares forms and files all patient information in a designated location
- Communicates clearly with provider staff and clinical support staff as appropriate
- Promotes public relations through prompt and courteous service
- Maintains respect for privacy by adhering to HIPAA confidentiality in all phases of work
- Works in conjunction with other Medical Providers and all other staff to ensure that the patients obtain the highest standards of integrated healthcare
Qualifications
Education:
- High school diploma or GED required
- Credentialed Medical Assistant, Certified Nurse Assistant, or Pharmacy Technician preferred, but others able to meet requirements are encouraged to apply
Experience:
- Related experience or knowledge preferred
Licensure/Certification:
- None
Office/Remote:
- Potential flexibility of office/home based on performance and clinic needs
Knowledge, Skills, and Abilities:
- Ability to detect errors in patient identification and make corrections
- Microsoft Programs: Excel, Word, and Outlook
- Knowledge of medical/dental terminology is required
- Bilingual English/Spanish preferred
- Sensitivity to low income and ethnic minority communities is a must
