Job Description
Due to continued growth, Meridian HealthCare, a large non-profit integrated healthcare agency with over 250 employees and 12 locations in Mahoning and Trumbull counties, is searching for a Licensed Therapist to join our TASC team.

Therapist Responsibilities and Duties:
- Interviews clients, coordinates assessments, and prepares a narrative summary/bio including problem areas, client strengths, treatment recommendations and diagnosis at TASC, juvenile detention center, and/or the county jail.
- Recommends level of care for chemical dependency treatment, ensures that client rights are upheld during the assessment process, and facilitates clients' active participation in treatment planning whenever possible. Informs clients of alternative treatment services and procedures that are available.
- Develops an initial treatment plan for each client accepted for treatment.
- Discharges clients that are accepted in services but who do not attend beyond the assessment process.
- Makes referrals and provides necessary case management consultation, correspondence and advocacy to optimize client's accessibility to needed services that are not provided by the Agency.
- Ensures that assessment data is complete, thorough, and correct before being forwarded to Clinical and Administrative Supervisors.
- Facilitates group counseling and completes appropriate and timely documentation.
- Provides individual counseling, updates Behavioral Health Service Plan as needed and in a timely manner, and completes appropriate and timely documentation.
- Maintains confidentiality standards with regard to client service records and client identifying information according to state, federal and agency guidelines.
Qualifications:
Master's Degree and Ohio licensure (LPC/LPCC) or Bachelor's Degree and Ohio Licensure (LSW) required. Competency or training in chemical dependency treatment preferred. Interns welcomed to apply as we will also consider Counseling/Social Work students close to graduation.
