Counselor – Mental Health
Job Description:
Provides orientation, assessment, treatment planning, individual and group counseling, education/information, case management, referrals, and transportation to mental health clients living in a group home. Works one on one with our clients to develop an Individualized Service Plan that is timely and based on client-identified goals and objectives. Participates in the development and maintenance of a therapeutic community for our clients and addresses the needs of the residents that arise during work shift. Supervises and coaches clients in developing functional skills necessary for daily living such as personal grooming, cleaning, laundry, grocery shopping, meal preparation, and using public transportation. Facilitates trainings, workshops, group meetings, and recreational activities and spends time with clients. Maintains familiarity with all clients in the unit through participating in Case Reviews, reviewing communication logs, etc. Provides education and support to clients’ family and friends. Assists with the daily operation of the residence through light housekeeping, cooking, inventory, and grocery/supply shopping. Monitors the clients’ self-administration of medication, keeps accurate and thorough records, responds to emergencies, and works with clients to make appointments and obtain appropriate outside services such as medical or dental care, therapy, or financial assistance. Collaborates with other East House staff and external treatment providers to make referrals and/or coordinate services, working together to enable clients to transition to more independent living in the future. This East House program is licensed by the NYS Office of Mental Health (OMH).
Hours: 35 hours per week, Tuesday 3 PM to 10:30 PM, Wednesday 2 PM to 10:30 PM, Thursday 7:30 AM to Noon, Friday 3 PM to 10:30 PM, and Saturday 7:30 AM to 3:30 PM
. Requirements:- A high school diploma and 5 years of human services work experience OR
- An associate’s degree in human services, psychology, sociology, or social work plus minimum 3 years of human services work experience OR
- A bachelor’s degree in human services, psychology, sociology, social work or equivalent OR
- An equivalent combination of education and experience.
Valid NYS motor vehicle license, safe driving record, and an insured personal vehicle for use at work are required.
Compensation:
Wages are determined based on job-related education and experience.
