Coordinator APP, CHF (Congestive Heart Failure) Program

RWJBH is seeking an APP Congestive Heart Failure (CHF) Program Coordinator in the Department of Cardiology at the New Brunswick campus. This is a full-time, day position (40 hours per week).

Requirements/Qualifications:

• NJ APN License required
• 2 years acute care hospital experience required
• Cardiology or Heart Failure preferred
• Current BLS/ACLS required
• Acute care certification, FCCS certification preferred.
• Candidate must possess effective communication, organization, and time management skills.

Job Duties:

• The Heart Failure Program Coordinator is responsible and accountable for assuring the delivery of quality health care services to the Heart Failure patients.
• The program supports the needs of the Heart Failure patients by design, management and implementation of the Heart Failure Program that standardizes care and facilitates continuous improvement to achieve maximum quality outcomes throughout the continuum of care.
• The Heart Failure Coordinator works with the interdisciplinary team in the development, implementation and evaluation of data collection, analysis and reports according to established clinical practice guidelines and standards.
• The Heart Failure Program Coordinator will assist in professional education programs as requested.

• The Heart Failure Program Coordinator will provide support/resources to the Heart Failure patient population.
