Job Summary:
Partner with Community Leaders and internal team to plan, develop and administer outreach programs to improve the wellbeing of the community including health and social programs as well as other activities. Work closely with the community to aid in providing healthcare information to patients, facilitating access to care and services as appropriate, and developing relationships with community organizations to further achieve shared goals and interests.
Job Responsibilities:
Key areas of responsibility
- Provide support to Hartford Region Community Outreach coordinating meetings, providing data and reports, maintaining community relationships, etc.
- Represent Hartford Region at system level for health equity initiatives, participating, providing input and executing system wide programs locally
- Execute the strategic vision of Hartford Region's Community Outreach function through operationalizing programs and initiatives related to health and social interventions, marketing outreach, communication and partner coordination
- In a clinical environment, meet with patients taking vitals and assessing their medical and social needs; connect patients with needed services within their community. Flexible in approach and function effectively in a work environment where multitasking is necessary.
- Perform other related duties as required.
Qualifications
- Bachelor's Degree in health or related field
- Two years experience in a non-profit, community based environment.
- Experience and ability to research in and utilize various databases
- Bilingual preferred.
- RN or LCSW required.
